Iatrogenic urosepsis

Abstract

Urosepsis due to nosocomial infection caused by multi-drug-resistant Klebsiella, Proteus and Pseudomonas strains is an increasingly important problem in Western Germany. It is a complication of any procedure involving the genitourinary tract, particularly common after TUR of the prostate. Its incidence increases with the socalled prophylactic use of antibiotics. Its dangers are connected with the frequent development of septicemic shock, the mortality of which is still very high. Since the underlying cause is often obscured by ineffective antibiotic therapy, it is recommended to take blood and urine for culture in all patients with sudden hypotension before the first dose of antibiotic is administered. A practical guide to initial therapy for undiagnosed bacteremia is presented. Particular emphasis is given to constant monitoring of c.v.p. and a careful search for a surgically treatable cause.
