Résumé
On connaÎt depuis longtemps l'importance des premières heures qui suivent l'apparition des symptÔmes dans la prise en charge des coronariens affectés d'un infarctus aigu du myocarde. Cependant, malgré la diligence apportée dans les soins, une proportion impressionnante de patients décèdent avant de parvenir en milieu hospitalier. L'objet de ce travail est d'estimer à partir des données hospitalières et du Registre des décès, le nombre de cas de mort subite survenus en une année au Tessin. L'importance d'une telle estimation est prépondérante pour le calcul de l'incidence d'une maladie parfois foudroyante, tel l'infarctus aigu du myocarde, ainsi que pour l'évaluation des mesures de prise en charge immédiate dans notre système de santé.
Zusammenfassung
Seit langem ist bekannt, dass bei der Behandlung des Herzinfarkts die ersten Stunden nach Auftreten der Symptome entscheidend sein können. Allerdings stirbt ein beträchtlicher Anteil der Patienten bereits bevor sie ein Spital erreichen. Ziel dieser Arbeit ist es, aufgrund von Spitaldiagnosenregistern und Todesursachenstatistik diesen Anteil für den Kanton Tessin für ein Jahr zu schätzen. Eine solche Schätzung ist unerlässlich für die Berechnung der Inzidenz einer oft rasch tödlich verlaufenden Krankheit wie des Myokardinfarkts, anderseits auch für die Beurteilung von Organisationsformen von Notfalltransport und -behandlung in unserem Gesundheitswesen.
Summary
In the treatment of acute myocardial infarction, the first few hours following the onset of symptoms are often decisive as to the outcome. However, many patients die before reaching a hospital. We tried to estimate this proportion for one year for the canton of Tessin by studying hospital data and afficial death certificates. Such an estimation is most important for the measurement of the incidence of an often rapidly fatal disease like acute myocardial infarction, as well as for the evaluation of forms of emergency transport and care in our health system.
