Description et évaluation d'une activité de conseil et d'écoute menée par des infirmières de santé publique auprès d'apprentis et de gymnasiens vaudois: le programme «jeunesse et santé»

Résumé

Le Programme «Jeunesse et Santé» propose depuis 1983 diverses Prestations de santé aux apprentis et gymnasiens vaudois: dépistages, éducation pour la santé. Cet article décrit l'activité d'écoute, de soins et de conseils individuels que les infirmières ont menée dans le cadre de permanences dans chaque établissement. Durant 1 an et demi, 969 jeunes ont été rencontrés pour des motifs de nature médicale (40%), psycho-somatique (30%) ou psycho-sociale (30%). La majorité ont consulté spontanément (40%), ou suite à un examen de dépistage (40%). 297 adolescents ont été adressés pour une suite de traitement à des institutions spécialisées (médecins traitants, permanences, services sociaux, etc.). Une évaluation faite trois mois plus tard révèle que les jeunes consultants sont en large majorité satisfaits (>90%). Un sondage effectué auprès des institutions montre que les prises en charge pour affections psycho-somatiques et psycho-sociales sont moins facilement couronnées de succès. L'article conclut sur l'intérêt et les limites d'une telle recherche descriptive et évaluative.

Seit 1983 bietet das Programm «Jugend und Gesundhcit» den waadtländischen Lehrlingen und Gymnasiasten verschiedene Leistungen (z. B. Reihenuntersuchungen, Gesundheitserziehung) an. Der Artikelbeschreibt, wie die Krankenschwestern im Rahmen ihrer Tätigkeit in jeder Anstalt, die Jugendlichen angehört, gepflegt und persönlich beraten haben. Im Laufe von 18 Monaten wurde das Gespräch mit 969 Jugendlichen aufgenommen, und zwar aus ärztlichen Gründen (40%), psycho-somatischen Gründen (30%) oder psycho-sizialen Gründen (30%). Die Besprechungen fanden spontan (40%) oder nach einer Reihenuntersuchung (40%) statt. In 297 Fällen wurden Jugendliche zur Weiterbetreuung an Fachkräfte überwiesen (Ärzte, Permanenzen, Sozialdienste usw.). Eine nach drei Monaten gemachte Auswertung zeigt, dass die grosse Mehrheit (über 90%) damit zufrieden ist. Eine Untersuchung bei den Fachkräften oder Anstalten ergibt, dass die Betreuung für psycho-somatische oder psycho-soziale Leiden nicht ganz soviel Erfolg hat. Es wird noch erwähnt, dass eine solche Beschreibungs- und Bewertungsforschung wohl ihren Nutzen, aber auch ihre Grenzen hat.

Since 1983 apprentices and students 16–19 years of the Canton of Vaud (Switzerland) have been offered various activities in the frame of a school-based health promotion program called “Jeunesse et Santé” (screening, health education...). This paper focuses on the description and the evaluation of emergency care and counseling activities runned by nurses in each school. During one and a half year, 969 adolescents have presented themselves for medical reasons (40%), or for problems of psycho-somatic (30%) or psycho-social (30%) nature. Most of the pupils have consulted either spontaneously (40%) or after a screening procedure (40%). 297 youngsters have been sent to another institution for further evaluation and treatment (personal physician, outpatient clinic, social services). Three months after the first consultation, the nurses have contacted both the adolescents an the institutions for an evaluation: A large part of the pupils are satisfied with the help they have received (>90%). However the folow-up seems more difficult to organize and less effective for the adolescents that have presented themselves with psycho-social problems. The paper end with the advantages and the limitations of an ambulatory research runned in the frame of school health activities.
