Screening of hypertension in combination with mass-radiophotography in Bern, Switzerland

Résumé

Un programme combiné — radiophotographie et mesure de la pression artérielle — pour le dépistage précoce des cas d'hypertension, qui sont adressés pour traitement au médecin de famille, a été soumis à une étude prospective. La cohorte examinée comportait 9493 sujets âgés de plus de 30 ans, actifs professionnellement. Le premier examen a décelé une proportion importante d'hypertensions inconnues, et peu de traitements suffisants. Lors d'un second examen, pratiqué en moyenne 2,4 ans plus tard, la proportion des hypertensions connues et des traitements suffisants au sein du groupe des hypertendus, était nettement plus élevée. La comparaison avec un groupe de contrôle examiné une seule fois montre, également au sein de la population générale, une tendance à mieux contrôler la pression artérielle; cette tendance spontanée est moins marquée que l'impact de la combinaison radiophotographie et mesure de la pression artérielle.

Summary

A programme for screening of hypertension in combination with mass-radiophotography is presented and evaluated in a prospective study. The population with two measurements (intervention-cohort) consists of 9'493 employed men and women aged 30 or more years. At the first measurement the precence of hypertension is frequently ignored and only a few are adequately treated. After about 2,4 years at the second measurement the number of unknown hypertensives has diminished and that of adequately treated hypertensives has increased significantly. Comparison with a similar group examined for the first time in 1980 showes that improvement in the intervention-cohort is more pronounced than in the general population.
