Summary
Anaesthetists are not rarely surprised by complications during anaesthesia, of which they cannot prevent death. The following results of the postmortem examination and also chemical or toxicological researches do not give any adaequate aetiologic and pathogenetic explanation.
In 3 cases of postmortem examination it is demonstrated, that complications can already occur in the beginning of anaesthesia, if the patient is suffering from a myopathy: for example an extreme elevation of body temperature, generalized spasmodic states of the cross-striped muscular system and also irreversible cardiac arrest and cessation of breathing.
This syndrome is called malignant hyperthermia respectively hyperpyrexia and hyperrigidity. In most cases it occurs right after the usual dosage of halothane and succinylcholine. The prooved heredity of this special form of reacting to narcotics and relaxants points to the same or resembling gene-dependent defects of molecules, as they are supposed for the aetiology of myopathies.
Also if there has not been known a manifest myopathy, the sceletal muscle should be examined by routine in those cases of fatal anaesthesia, which are characterized by hyperthermia or hyperrigidity.
The PAS-reaction and especially Goldner's method of staining simplify the diagnostic, they can also be recommended for bioptical examinations in clinical cases of uncertain myopathy. Patients with myopathy particulary young people or infants must be regarded as only in some respects “narcotisable”. It is supposed, that these patients show a changed pharmacokinetic upon narcotics and relaxans.
Zusammenfassung
Patienten mit einer Myopathie, besonders Jugendliche und Kinder, müssen als nur bedingt narkosefähig bezeichnet werden, da bei ihnen offenbar eine veränderte Pharmakokinetik von Narkosemitteln und Relaxantien besteht.
An Hand von 3 Obduktionsfällen wird demonstriert, daß bei Patienten, die an einer Myopathie leiden, Komplikationen schon zu Beginn einer Narkose auftreten können, z. B. extreme Erhöhung der Körpertemperatur, generalisierte Krampfzustände der quergestreiften Muskulatur sowie irreversibler Herz- und Atemstillstand.
Dieses Syndrom wird als maligne Hyperthermie bzw. Hyperpyrexie und Hyperrigidität bezeichnet, es tritt meist unmittelbar nach üblicher Dosierung von Halothan und Succinyl auf. Die nachgewiesene Erblichkeit dieser besonderen Reaktionsform auf Narkosemittel und Relaxantien spricht für gleichartige oder ähnliche Gen-abhängige Moleküldefekte, wie sie ätiologisch für Myopathien angenommen werden.
Selbst wenn klinisch vor der Narkose keine manifeste Myopathie bekannt geworden ist, sollte man routinemäßig bei der Aufklärung tödlicher Narkosezwischenfälle, die durch Hyperthermie oder Hyperrigidität gekennzeichnet sind, Skeletmuskulatur histologisch untersuchen. Neben der PAS-Reaktion erleichtert insbesondere die Goldner-Färbung die Diagnostik. Sie wird auch für bioptische Untersuchungen bei der klinischen Absicherung der Diagnose Myopathie empfohlen.
