Abstract
The World Health Organization has taken the lead in recent years in encouraging an appropriate focus on palliative care which includes but goes beyond terminal care. Comprehensive care of patients with advanced progressive disease requires increased emphasis, with adequate resources. Management of major symptoms is a crucial component of such a care program and requires the application of recent advances in palliative therapeutics (notably but not only good quality pain management), whether or not direct antitumor strategies are being continued. The choice of treatment approaches must be related to the patient's personal priorities.
Résumé
L'Organisation Mondiale de la Santé a, ces dernières années, montré la voie à suivre en encourageant les démarches justifiées de soins palliatifs qui comprennent mais vont au delà des seuls soins en phase termanale. L'ensemble des soins donnés aux patients atteints de maladies très évoluées requiert une grande énergie et des ressources suffisantes. La prise en charge des principaux symptômes est une des composantes essentielles d'une telle démarche de soins. Elle nécessite la mise en oeuvre des progrès récents en matière de traitements palliatifs (en particulier une prise en charge complète de la douleur), que les traitements à visée anti-tumorale soient poursuivis ou non. Le choix de la stratégie thérapeutique doit tenir compte des priorités personnelles du patient.
Resumen
Recientemente la Organización Mundial de la Salud ha asumido liderazgo en cuanto a estimulan el interés en el cuidado paliativo de pacientes con cáncer avanzado y progresivo, el cual incluye cuidado terminal, pero que va más allá del mismo. El cuidado comprensivo de pacientes con enfermedad avanzada y progresiva requiere un mayor y énfasis adecuados recursos. El manejo de los síntomas principales es un componente crucial de un programa comprensivo, y exige la aplicación de los avances recientes en terapia paliativa (especial, pero no exclusivamente, atención del dolor) así se mantengan o no regímenes de terapia directa antitumoral. La escogencia de los enfoques terapéuticos deben hacerse en relación con las prioridades personales del paciente.
