Abstract
The personal experience with 5 rare types of malignant melanoma is reviewed to point out some of the practical problems in the diagnosis and management of these tumors. The rare forms discussed are conjunctival, nasal, oral, vulvar, and penile melanomas. All pigmented lesions in the oral cavity, but not the penis or vulva, should be prophylactically excised as lesions in the mouth have a higher malignant potential. Local excision of all 5 forms of primary melanomas, no matter how locally advanced they may be, is the sole treatment. Nevertheless, anatomic constraints often preclude surgery with generous margins and consequently local recurrence, particularly for conjunctival, nasal, and oral primary lesions, is usually the major first failure in treatment. Lymph node dissection is only performed if the regional nodes are palpable at the time of first presentation. Elective lymph node dissections are not performed since the patients are often elderly, the lymphatic drainage is usually ambiguous or multiple, and the disease tends to spread hematogenously rather than lymphatically. Surgery still remains the cornerstone of treatment for these rare forms of melanoma but prognosis is very poor since surgery is often a palliative rather than a curative measure. Improved survival may depend on identifying more effective chemotherapeutic and immunologic agents.
Résumé
L'expérience personnelle de l'auteur concernant cinq types rares de mélanome malin a été revue avec l'intention de mettre en évidence les problèmes diagnostiques et le traitement de ces tumeurs. Ces formes rares sont les mélanomes de la conjonctive, du nez, de la bouche, de la vulve et de la verge. On pense que toute lésion pigmentée de la bouche doit être excisée, vu son potentiel élevé de dégénérescence. L'excision locale de ces 5 formes de mélanomes malins primitifs, quel que soit leur stade, est le seul traitement réellement efficace. Néanomoins, des contraintes anatomiques sont parfois incompatible avec une chirurgie large, notamment en ce qui concerne les lésions primitives conjonctivales, nasales et orales. La lymphadénectomie régionale est envisagée lorsque les adénopathies sont palpables lors du premier examen. La lymphadénectomie életive n'est pas souvent réalisée car les patients sont âgés, le drainage lymphatique mal défini ou multiple et la dissémination hématogène plutôt que lymphatique. La chirurgie reste la base du traitement de ces formes rares, mais le pronostic est mauvais car la chirurgie n'est souvent que palliative. Une amélioration de la survie pourrait découler d'une meilleure approche thérapeutique associant chimio et immunothérapie.
Resumen
Se revisa le experiencia pesonal de uno de los autores con 5 tipos raros de malenoma maligno con el propósito de resaltar algunos de los problemas prácticos en el diagnóstico y manejo de estos tumores. Las formas raras que aquí se discuten son los melanomas conjuntivales, orales, vulvares y peneanos. Se considera que todas las lesiones pigmentadas de la cavidad oral, pero no del pene o de la vulva, deben ser resecades en forma profiláctica, puesto que las lesiones de la boca poseen un mayor potencial de malignidad. Las rescessión local de las 5 formas de melanoma primario, no importa que tan avanzadas estén, representa el único tratamiento apropiado. Sin embargo, limitaciones de tipo anatómico con frecuencia impiden realizar la resección quirúrgica con márgenes generosos, por lo cual la recurrencia local, especialmente en el csao de las lesiones primarias conjuntivales, nasales y orales, aparece como la principal forma de falla del tratamiento. La disección ganglionar se realiza sólo si los ganglios linfáticos regionales son palpables en el momento de la primera presentación del paciente. No se realizan disecciones ganglionares electivas puesto que los pacientes con frecuencia son de edad avanzada, el drenaje linfático usualmente es ambiguo o múltiple y la enfermedad iende a propagarse por vía hematógena más que linfática. La cirugía todavía se mantiene como la clave del tratamiento de estas formas raras de melanoma, pero el pronóstico es muy pobre pues la operación con frecuencia resulta ser una medida paliativa más que curativa. Le mejoría en las tasas de sobrevida depende del desarrollo de agentes quimioterapéuticos e inmulógicos más eficaces.
