Bleeding varices of the small bowel as a complication of pancreatitis: Case report and review of the literature

Abstract

A case of a 48 year old male with a history of alcohol abuse, chronic relapsing pancreatitis, and massive hemorrhage into the small intestine is reported. The patient had previously undergone a cholecystojejunostomy. Imaging studies demonstrated occlusion of the splenic, superior mesenteric, and distal portal veins with large varices in the jejunum. He recovered following jejunal resection and Roux-en-Y cholecystojejunostomy. The mechanism for formation of varices in the small bowel in this clinical setting is discussed.

Résumé

Le cas d'un homme de 48 ans, grand alcolique, et connu comme ayant une pancréatite chronique récidivante, a présenté une hémorragie intestinale basse grave. L'imagérie a mis en évidence une occlusion complète des veines splénique, mésentérique supérieure et portes distales, associée à des varices importantes au niveau du jéjunum. Une résection jéjunale et une anastomose cholécysto-jéjunale ont été réalisées avec succès. Le mécanisme de formation de varices dans cette situation est discuté.

Se informa el caso de un hombre de 48 años con historia de abuso de alcohol, pancreatitis crónica recurrente y hemorragia masiva en el intestino delgado; el paciente había sido sometido a colecistoyeyunostomía unos años antes. Las imágenes diagnósticas demostraron oclusión de las venas espléncia, mesentérica superior y portales distales, con grandes várices en el yeyuno. Se realizó resección yeyunal con colecistoyeyunostomía de Roux-en-Y y esplenectomía; el paciente se recuperó, después de algunas complicaciones. Este caso representa el primer informe de sangrado de várices del intestino delgado como complicación de una pancreatitis. Se discute el mecanismo de la formación de las várices intestinales en tal situación clínica.
