New diagnostic imaging in rectal cancer: Endosonography and immunoscintigraphy

Abstract

Endosonography of the rectum, combining the advantages of both endoscopy and sonography, provides information not at present available from other imaging modalities. It is remarkably accurate in the pre-operative assessment of tumor penetration. In the evaluation of lymph nodes, the specificity and sensitivity is 83% and 72%, respectively. For the detection of local recurrence in asymptomatic patients promising results can be obtained. In contrast, the potential benefits of immunoscintigraphy in patients with rectal cancer have yet to be determined but preliminary results indicate that immunoscintigraphy in combination with endosonography may assist the surgeon in confirming or excluding the suspicion of recurrence.

Résumé

L'échoendoscopie du rectum, associant les avantages combinés de l'endoscopie et de l'échographie, fournit des reseignements précieux, impossibles à obtenir par les autres procédés d'imagerie actuels. Cet examen est remarquement précis dans le bilan de l'extension préopératoire de l'invasion tumorale. Dans l'évaluation des adénopathies, la spécificité et la sensibilité étaient respectivement de 83 et 72%. Cet examen fournit également des renseignements intéressants en cas de récidive. L'intérêt de l'immunoscintigraphie, par contre, reste à déterminer, mais les résultats préliminaires semblent indiquer qu'à l'avenir, cet examen, combiné avec l'échoendoscopie, peut confirmer ou infirmer tout soupçon de récidive.

Resumen

La endosonografía rectal que combina las ventajas de la endoscopia con las de la sonografía, provee información que actualmente no se logra con otras modalidades de imagenología. Es notoriamente certera en la valorización preoperatoria del grado de penetración tumoral. En la evaluación de los ganglios linfáticos, la especificidad y la sensibilidad han probado ser 83% y 72%, respectivamente. Promisorios resultados podrían ser logrados en la detección de las recurrencias locales en pacientes asintomáticos. Por el contrario, los beneficios potenciales de la inmunocintigrafía en pacientes con cáncer rectal aún no han sido determinados, pero los resultados preliminares indican que la inmunocintigrafía en combinación con la endosonografía puede ayudar al cirujano en la confirmación o la exclusión de una posible recurrencia.
