Zusammenfassung
Es wird über ein Steuerungsverfahren zum Betrieb von implantierbaren Hilfsventrikeln (Bypass) berichtet, das zur physiologischen Triggerung den P-Wellenimpuls des epikardial abgeleiteten Elektrokardiogramms ausnutzt. Mit Hilfe dieses gekoppelt elektromechanisch-hydraulischen Steuersystems wurden hämodynamische Untersuchungen über den Wirkungsgrad derartiger Hilfspumpensysteme hinsichtlich der mechanischen Entlastung des Herzens und verbesserter Coronardurchblutung durchgeführt. Maximale Entlastung und optimale Coronardurchblutung ergeben sich, wenn zwischen Herz- und Hilfspumpensystole eine zeitliche Verschiebung von 320–360 ms eingestellt wird.
Summary
A control unit for implantable left heart-bypass-pumps is described. This electromechanic-hydraulic device is triggered by the P-wave of the epicardial electrocardiogram. Hemodynamic studies have been performd with this system during bypass in order to evaluate the effect on ventricular work load and on coronary perfusion. The most effective left ventricular decompression and the highest coronary perfusion rate was obtained with an interval of 320 to 360 ms between the systole of the heart and the one of the bypass pump.
