Modifizierte Technik der isotopen Pankreastransplantation

Zusammenfassung

Bei der isotopen Homotransplantation des Pankreas treten verschiedene verhütbare Komplikationen auf. Die Insuffizienz des Choledochusstumpfes wird durch Einpflanzung des Choledochus in das Duodenum, die aszendierende Cholangitis durch Benützung erst der zweiten Jejunumschlinge für die distale Anastomosierung mit dem Transplantat und die ulceröse Duodenitis wenigstens teilweise durch Magenresektion beim Empfänger behoben. Durch Belassung der A. pancreaticoduodenalis inferior kann der Proc. uncinatus in jedem Fall mittransplantiert werden. Zum Studium des Kohlenhydratstoffwechsels wird zum Teil anstatt der routinemäßigen venösen Anastomose mit der V. cava die allerdings komplikationsreichere Anastomose mit der V. portae durchgeführt.

In experimental isotopic homotransplantation of the pancreas including the duodenum various complications were observed which can be obviated by technical refinements. Implanting the stump of the common bile duct in the duodenum instead of ligaturing it prevents stump necrosis and insufficiency. Ascending cholangitis can be prevented by implanting the graft more deeply, i.e. in the second loop of the jejunum. The ulcerous duodenitis observed in the animals surviving for longer periods can be influenced by gastric resection in the host. If the A. pancreaticoduodenalis inferior is left on the transplant, the amputation of the uncinate process of the pancreas, which is otherwise sometimes necessary although not of significance, can be obviated. For carbohydrate metabolism studies the routine venous anastomosis with the V. cava can be replaced by anastomosis with the V. portae, although this involves complications.
