Zusammenfassung
Die Abnahme der Inzidenz des Magenkarzinoms bei gleichbleibender oder leicht zunehmender Inzidenz des Dickdarmkarzinoms spricht für eine unterschiedliche Ätiologie dieser beiden Formen des Krebses. Durch eine Befragung von 50 Patienten mit Magenkrebs, 100 Patienten mit Dickdarmkrebs und 150 gleichaltrigen gesunden Kontrollpersonen wurde versucht, anamnestische Unterschiede und somit Anhaltspunkte in bezug auf eventuelle ätiologische Faktoren dieser beiden Formen des Krebses zu ermitteln.
Bei den Patienten mit Dickdarmkrebs war die familiäre Häufung des Polyposis deutlich erkennbar. Ferner wurden von diesen Patienten Appendektomien, Hämorrhoiden und unregelmässige Darmtätigkeit deutlich häufiger erwähnt als von den Kontrollpersonen. Die Patienten mit Magenkrebs unterschieden sich anamnestisch kaum von den Kontrollpersonen.
Retrospektive Erhebungen über Ernährungsgewohnheiten scheitern an den ungenauen Angaben der Befragten in bezug auf Art und Menge der durchschnittlich konsumierten Nahrungsmittel. Prospektive Erhebungen bei Bevölkerungsgruppen mit spezifischen und wenig veränderlichen Ernährungsgewohnheiten dürften oher zu positiven Resultaten führen, sind jedoch mit einem viel grösseren Aufwand verbunden.
Résumé
La diminution de l'incidence du cancer de l'estomac et la relative stabilité ou même la légère augmentation des carcinomes du gros intestin plaident en faveur d'une étiologie différente de ces deux formes de cancer. En questionnant 50 patients atteints du cancer de l'estomac, 100 autres souffrant du cancer du gros intestin, et un groupe de contrôle de 150 personnes d'âges équivalents, on a tenté de trouver, au niveau de l'anamnèse, des différences qui seraient en même temps des points de référence concernant d'éventuels facteurs étiologiques de ces deux formes de cancer.
Chez les patients atteints du cancer du gros intestin, la forte incidence familiale de polypose a pu facilement être mise en évidence. De plus, on trouve plus souvent chez ces patients que dans le groupe de contrôle des appendicectomies, hémorroïdes et irrégularités intestinales. Au niveau de l'anamnèse, les patients souffrant du cancer de l'estomac ne se distinguent guère du groupe de contrôle.
Des études rétrospectives concernant les habitudes nutritionnelles échouent à cause de l'imprécision des réponses données quant au type et à la quantité des aliments consommés en moyenne. Des études prospectives chez des groupes ayant des habitudes nutritionnelles spécifiques et peu variables devraient donner des résultats plus positifs, bien que de telles enquêtes nécessitent des investissements beaucoup plus considérables.
Summary
The decrease in incidence of stomach cancer in the face of a constant or slightly increasing incidence of cancer of the colon suggests a different aetiology for these two forms of cancer. Through questioning 50 patients with cancer of the stomach, 100 patients with cancer of the colon and 150 age-matched healthy controls, an attempt was made to discover differences in regard to their history and thus find clues concerning possible aetiological factors in the causation of these two forms of cancer.
A familial aggregation of polyposis was clearly demonstrable amongst the patients with cancer of the colon. These patients mentioned appendectomies, piles and irregular intestinal movements distinctly more frequently than the controls. Patients with stomach cancer did not differ from controls.
Retrospective ascertainment of eating habits failed, because of imprecise statements concerning kind and quantity of habitually consumed foods. Prospective studies amongst population groups with specific and fairly constant eating habits would be more likely to yield positive results, but require much greater expenditure and effort.
