Summary
Comparison of the initial situation and the conceptual treatment goal determines which force system will be necessary or at least advantageous for the resolution of an orthodontic problem. Control of the full range of orthodontic tooth movements requires control over all axes of space and the possibility of torquing combinations in all planes. The use of a fixed appliance is strongly indicated if the planned arrangement of the dental units can only be attained by three dimensional control. Although the full biomechanical potential may not be exploited, this appliance is preferred by many clinicians in those situations where its superior efficiency and precision are heeded. The main field of action is within the dental arch: in the mixed dentition to create optimal conditions for the further developmental stages; in the permanent dentition to execute exact arrangement of the dental units. The associated goal is intermaxillary coordination to fulfill the dynamic requirements of occlusion. The indication for fixed appliance treatment therefore depends on the type and severity of intra- and intermaxillary deviations to be resolved.
