Réflexions après la fermeture d'un centre de bilans de santé

Résumé

Les conditions de réussite d'un centre de bilans de santé sont analysées à la suite d'une réalisation faite en milieu hospitalier. Le centre ne doit pas être intégré à un centre de soins actifs. Il doit répondre aux critères de la tragédie classique: Unité de lieu: toutes les opérations doivent être faites dans un lieu bien défini; unité de temps: tout doit être fait dans une durée prédéterminée et aucune dérogation ne doit être admise; unité d'action: tout le personnel doit avoir pour but de faire de la médecine préventive.

Trois autres conditions pour réussir: avoir une population surveillée stable pour pouvoir la suivre longtemps; avoir un excellent contact avec les médecins traitants pour obtenir de ceux-ci le retour de leurs constatations médicales au centre; enfin il est nécessaire de ne pas concevoir le bilan de santé comme une fin en soi mais bien comme une technique médicale parmi les autres pour surveiller la santé d'une population.

Zusammenfassung

Gestützt auf ein im Rahmen eines Spitals durchgeführten Projekts werden die zum Erfolg eines Vorsorgeuntersuchungszentrums notwendigen Bedingungen analysiert. Das Zentrum sollte nicht in den üblichen Spitalbetrieb integriert werden. Es sollte die Bedingungen der klassischen Tragödie erfüllen: Im Sinne der Einheit des Ortes sollten alle seine Tätigkeiten am gleichen, genau bestimmten Ort durchgeführt werden; im Sinne der Einheit der Zeit sollte ausnahmslos alles innerhalb einer vorbestimmten Zeit zum Ablauf kommen; und im Sinne der Einheit der Handlung sollte das ganze Personal zum Zwecke einer präventivmedizinischen Tätigkeit angestellt sein.

Drei weitere zum Erfolg notwendige Bedingungen sind zu erfüllen: die betreute Bevölkerung sollte eine genügende Stabilität aufweisen, um über längere Zeit überwacht werden zu können; der Kontakt mit den behandelnden Ärzten sollte ausgezeichnet klappen, um sicherzustellen, dass ihre medizinischen Feststellungen zum Zentrum zurückgelangen; schliesslich ist es notwendig, dass die Gesundheitsvorsorgeuntersuchung nicht als Selbstzweck konzipiert ist, sondern als eine von mehreren medizinischen Techniken zur Überwachung der Gesundheit einer Bevölkerung.

Summary

The conditions for success of a screening center are analyzed following the realization of a project within a hospital. The center should not be integrated into an active therapeutic center. It should follow the criteria of the classical tragedy. Unity of place: all operations of the center should take place within well defined localities; unity of time: all should be completed without exception within a predetermined time period; unity of action: all personnel should have the only aim of doing preventive medical work.

There are three further conditions for success: surveillance of a stable population that can be followed for a long time; an excellent contact with the treating physicians in order to assure the return to the center of their medical observations. Finally, the screening examination should not be considered as an aim in itself, but a medical technique among others to conduct a surveillance of a population.
