Zusammenfassung
Vor zehn Jahren wurde im Rahmen des Kaiser-Permanente-Krankenversicherungsplanes das erste «automatisierte» Screening-Projekt eröffnet. Es sollte als Schaltstation für Gesunde, um ihre Gesundheit Besorgte, Frühkranke und Kranke einen optimalen Einsatz der Mittel garantieren. Ein eindrucksvolles Untersuchungsprogramm liefert Resultate, die vom Computer direkt verarbeitet und zusammengestellt werden können und dem Arzt einen wertvollen Überblick über den Gesundheitszustand des Patienten geben. Das neue Untersuchungssystem ist heute von Behörden, Bevölkerung und Ärzteschaft der USA akzeptiert, obwohl keine wissenschaftliche Beweise für die Wirksamkeit vorliegen. Sein Nutzen dürfte aber von Ort zu Ort stark schwanken und sollte für jede Situation neu überdacht werden.
Résumé
Le premier projet d'un bilan de santé automatisé a été mis en pratique dans le cadre des assurances maladies Kaiser-Permanente. II devait orienter les individus sains, soucieux de leur santé, ceux dans un stade morbide précoce et les personnes malades vers les institutions indiquées pour leur état de santé. Une série impressionnante de mesures fournit des résultats susceptibles d'être classés directement par l'ordinateur et qui procure au médecin un véritable bilan de santé de son patient. Le nouveaux système d'examens de santé est à l'heure actuelle admis par la population, les autorités et le corps médical des Etats-Unis, quand bien même il n'existe pas de preuve scientifique de son efficacité. Toutefois son utilité différera suivant les lieux et devrait être réexaminée dans chaque cas particulier.
Summary
Ten years ago the first automated multiphasic health testing faculty opened its doors for enrollees of the Kaiser-Permanente-Health Insurance Plan. It should channel healthy, worried well, early sick and sick individuals to appropriate institutions for an optimal use of all health resources. An impressing set of examinations yields results for direct processing and compilation by a computer to provide the physician with useful data about the health of his patient. The new system of medical examination has been accepted by authorities, population and physicians of the U.S. although only little scientific evidence for efficacy and efficiency does exist. The utility of such facilities varies from situation to situation and should be evaluated carefully before embarking on similar programs.
