Filteruntersuchungen im Vorschul- und Schulalter

Zusammenfassung

Die Schulkinder des Kantons Basel-Stadt werden im Kindergartenalter, im vierten und im achten Schuljahr durch eine multidisziplinäre schulärztliche Routineuntersuchung erfasst, an der Ärzte, Psychologen und Logopäden mitwirken. Von insgesamt 7500 untersuchten Kindern werden etwa 1500 zur weiteren Abklärung und Behandlung an Hausarzt, Kinderarzt und Spezialisten überwiesen. Neben der klassischen klinischen Untersuchung erweist sich die Erfassung von Risikofaktoren im psychischen und sozialen Bereich als immer bedeutender. Mit auf elektronischer Datenverarbeitung basierenden epidemiologischen Studien wird versucht, Grundlagen für eine kontinuierliche Anpassung an die Bedürfnisse unserer Bevölkerung zu schaffen. Konflikte mit anderen Wertvorstellungen — wie Bewahrung der Privatsphäre — lassen sich dabei nicht vollständig vermeiden, bei genügendem Verständnis für die fachlichen, technischen und politischen Aspekte aber durchaus lösen.

Les enfants du canton Bâle-Ville sont examinés de routine à l'âge de l'école enfantine, de la 4e et de la 8e année scolaire. Ces examens sont faits en collaboration par des médecins, des psychologues et des logopédistes. Du total des 7500 enfants examinés, environ 1500 sont déférés au médecin de famille, au pédiatre et aux spécialistes pour évaluation supplémentaire et traitement. A part l'examen clinique l'évaluation des facteurs de risque dans le domaine psychique et social prend toujours plus d'importance. Avec les études épidémiologiques basées sur la programmation électronique on essaie d'établir des fondements pour une adaptation continuelle aux besoins de la population. Des conflits avec d'autres systèmes de valeur tel que la protection de la sphère privée sont inévitables. Mais ces problèmes peuvent être résolus avec suffisamment de compréhension pour les aspects scientifiques, techniques et politiques de tous les intéressés.

Summary

The children of the Swiss canton of Basle-City are routinely examined in the kindergarten-age, the fourth and the eighth grade. Physicians, psychologists and logopedicians work together in these examinations. Of a total of 7500 children about 1500 are referred to their family physician, to pediatricians or other specialist for further diagnostic evaluation and treatment. In addition to the clinical examination, the evaluation of risk factors — many of them in the psycho-social realm — becomes more and more important. Based on electronic data processing, epidemiologic studies create the basis for a continuous improvement of the program of the school health office to meet the needs of the population. Conflicts with other value-systems — regarding e.g. the protection of privacy — cannot always be avoided, but they can be solved if all parties have enough understanding of scientific, technical and political factors.
