Résumé
La nécessité d'une détection précoce des handicaps du nourrisson et de l'enfant est soulignée. Les différentes modalités pour parvenir à ce but sont discutées et l'expérience du service de pédiatrie de l'Hôpital cantonal de Lausanne dans le contrôle systématique des nouveau-nés et dans la surveillance à long terme des enfants à risque périnatal élevé, est rapportée. Les conclusions sont que le groupe des enfants à haut risque périnatal nécessite une surveillance spéciale par une équipe médicosociale bien formée et qu'un effort de généralisation des contrôles systématiques de tous les nourrissons doit être entrepris, particulièrement dans les zones rurales et montagnardes. Quelques suggestions pour l'établissement d'un tel programme sont faites.
Zusammenfassung
Die Notwendigkeit einer frühzeitigen Diagnose der Anomalien des Säuglings wird betont. Die verschiedenen Möglichkeiten, um dieses Ziel zu erreichen, sowie die Erfahrung der pädiatrischen Klinik des Kantonsspitals Lausanne mit der systematischen Kontrolle des Neugeborenen und mit der langdauernden Beobachtung der Risikokinder werden beschrieben. Die Schlussfolgerungen zeigen, dass die Risikokinder eine besondere Beobachtung durch eine ausgebildete sozialmedizinische Gruppe benötigen. Dazu muss der Versuch einer Verbreitung der systematischen Kontrollen sämtlicher Säuglinge unternommen werden. Einige Anregungen für die Aufstellung eines solchen Programms werden gemacht.
Summary
The need for an early detection of handicaps in infancy is stressed. The idea of early recognition is discussed from various viewpoints. The experience of the Department of paediatrics in Lausanne is given, and the systematic examinations of all new-borns as well as the follow-up examinations of the high risk neonates is described. It is concluded that the emphasis must be on systematic surveillance of all children and that a few “high risk” babies require special supervision. Practical suggestions are given for establishing such a programme.
