Zur Frage der Semimalignität bei Mischtumoren der großen Speicheldrüsen

Zusammenfassung

Bei einer histologischen Nachuntersuchung von Tumoren in der gl. parotis und gl. submandibularis zeigte es sich, daß in 681 von 1019 Fällen ein echter Mischtumor vorlag. Eine Korrelationsuntersuchung zwischen der histologischen Struktur der Mischtumoren und dem klinischen Verlauf wurde vorgenommen. Hierbei ergab sich, daß die meistens als semimaligne bezeichneten Kriterien, wie Zellreichtum, Dominanz der epithelialen Komponente, cylindromatöse Strukturen, multiple Herde und unzureichende Kapselabgrenzung, keine Malignität in Form von erhöhter Rezidivfrequenz, Metastasen und Tumormortalität ergeben. Nur bei klarem infiltrativ-destruktivem Wachstum eines Tumors in das umgebende Gewebe konnten Metastasen und Tumormortalität beobachtet werden. Die Mischtumoren können daher in zwei Gruppen eingeteilt werden;benigne Tumorenohne infiltrativ-destruktives Wachstum (ca. 97%) undmaligne mit solchem Wachstum.
