Zusammenfassung
Die Probleme der angiographischen Bestimmung der diastolischen Druck-Volumen-Beziehung (P-V-B) wurden am Beispiel der durch eine ventrikuläre Extrasystole verlängerten Diastole und zweier aufeinanderfolgender Herzzyklen ohne Intervention aufgezeigt. Dazu wurden bei 16 Patienten mit funktionsgestörtem linken Ventrikel biplane Cineventrikulogramme mit simultaner Druckmessung angefertigt.
Die Druck-Volumen-Beziehung wurde mit dem Ansatz von Diamond und Forrester (p=b·ea.v+c) (Ansatz [1]) und mit dem üblicherweise benutzten vereinfachten Ansatz (p=b'·ea'.v) (Ansatz [2]) ermittelt. Die nach Ansatz (1) bestimmten Kurvenparameter b, a und c zeigten signifikante, jedoch ungerichtete Veränderungen. Dies gilt sowohl für zwei aufeinanderfolgende Diastolen als auch für die postextrasystolisch verlängerte Diastole. Die nach Ansatz (2) bestimmten Parameter b' und a' waren dagegen nicht signifikant verschieden.
Die Fehlerquadratsummen für die Approximation nach Ansatz (1) lagen im Mittel um 8% niedriger als für den Ansatz (2), d. h.: die nach Ansatz (1) ermittelte Kurve berücksichtigt die gemessenen Druck-Volumen-Wertepaare genauer.
Zwei Druck-Volumen-Beziehungen mit identischen Steigungen, aber unterschiedlichem Druckniveau können mit Ansatz (2) nicht erfaßt werden, da hierbei die Krümmung der Druck-Volumen-Kurve in Abhängigkeit vom Druckniveau ermittelt wird.
Die Befunde zeigen, daß die Anwendung des vereinfachten Ansatzes (2) zu systematisch falschen Kurvenparametern und damit zu scheinbaren Änderungen der elastischen Eigenschaften des linken Ventrikels führen kann. Durch Anwendung des Ansatzes (1) wurde gezeigt, daß eine exakte Beschreibung der Druck-Volumen-Beziehung mit angiographisch ermittelten Volumina nicht möglich ist.
Summary
The problems of determination of the left ventricular diastolic pressure-volume relationship were demonstrated by analysis of post-extrasystolic beats and of two consecutive normal beats. Quantitative high-speed left ventriculography with simultaneous pressure recording was performed in 16 patients with reduced left ventricular function. The diastolic pressure-volume relationship was evaluated using the formula of Diamond and Forrester (p=b·ea.v+c)(formula [1]) and a simplified expression (p=b'·ea'.v) (formula [2]), usually used. The parameters b, a and c of formula (1) showed significant but nonuniform alterations. The parameters b' and a' of formula (2) did not change significantly. The residual sums of squares of the approximation using formula (1) were about 8% less than those of formula (2); i.e.: the measured left ventricular pressures and volumes could be fitted more exactly by formula (1). Two pressure-volume-curves with identical slopes but different operating pressure-levels could not be described by formula (2), because in this case the computed slope of the pressure-volume curve was influenced by shifts of the operating pressure-level. The results show that the diastolic pressure-volume relationship cannot exactly be described by formula (2). The application of formula (1) proved the angiographic determination of left ventricular volumes to be insufficient for the evaluation of the diastolic pressure-volume relationship.
