Summary
In 20 open-chest dogs intramyocardial pressure was measured simultaneously in two regions:

a)
in the normal myocardium (IMPsp)
-
b)
in a controlled perfused area of the myocardium, supplied by a cannulated coronary artery (IMPp). The preparation permitted the study of the influence of contractility, ventricular pressure and coronary blood flow on IMP.

A 47% change in coronary flow with a 26% change in coronary perfusion pressure results in changes of systolic and diastolic IMPp of 14% and 29% respectively. These effects can be explained by variations of intramural fluid content and, hence, of diastolic fiber length.

Changes of systolic ventricular pressure (mean 27%) by preload varations result in a mean change of systolic IMPsp of 19% and of systolic IMPp of 11%.

Changes of systolic ventricular pressure (mean 33%) by afterload variations augment IMPsp by 30% and IMPp by 13%.

The increases of IMPsp and IMPp induced by instantaneous changes of afterload are equal.
The most important determinants of IMP are myocardial contractility, coronary perfusion and intraventricular pressure. The influence of coronary perfusion and ventricular pressure on IMP is of the same order of magnitude.
Zusammenfassung
Bei 20 Hunden mit offenem Thorax wurde der intramyokardiale Druck simultan im spontan durchbluteten Myokard (IMPsp) und in einem isoliert perfundierten Bezirk (IMPp) gemessen. Damit war es möglich, den Einfluß von Kontraktilität, Ventrikeldruck und Koronardurchblutung auf den intramyokardialen Druck getrennt zu erfassen.

Änderungen des Perfusionvolumens um 47% bzw. des Perfusionsdruckes um 26% führen zu Änderungen des systolischen und diastolischen IMPp um 14% bzw. 29%. Dies wird durch eine geänderte diastolische Faserdehnung erklärt.

Durch Änderung der Vorbelastung bedingte Ventrikeldruckänderungen um im Mittel 27% führen zu Änderungen des systolischen IMPsp um 19% und des systolischen IMPp um 11%.

Durch Änderung der Nachbelastung bedingte Ventrikeldruckänderungen von 33% systolisch führen zu einem Anstieg des IMPsp um 30% und des IMPp um 13%.

Bei instantanen Erhöhungen der Nachbelastung ist die Zunahme von IMPsp und IMPp gleich groß.
Der IMP wird im wesentlichen von 3 Determinanten, Kontraktilität, Koronardurchblutung und Ventrikeldruck bestimmt. Dabei ist der Einfluß der Koronardurchblutung und des Ventrikeldruckes auf den intramyokardialen Druck etwa gleich groß.
