Summary
A method is theoretically described to test the accuracy of various lead system of the ecg. The principle of this method consists in the following procedure. The heart position is changed maximally in a proband by varying the position of the body and by deep inspiration and exspiration. If the lead field is parallel, the recorded vector loops should be conventible into each other by merely rotating them electronically, because the cardiac determinants of the veg will scarcely be changed by a change in heart position. The deformations of the veg in the edgeside and broadside view are an appropriate measure of the inhomogenities (the “accuracy”) of the lead field, mostly caused by the configuration of the chest and the inhomogeneous conductivity of the tissues and the resulting lack of parallelity in the lead lines of the field. Criteria to test the form of the vector loop are developed, measuring quantitatively the similarly of the loops recorded in different heart positions. The “best” lead ist regarded to be that with the greatest similarity of the loops in extreme dislocations of the heart.
Zusammenfassung
Es wird eine Meßmethode theoretisch entwickelt, um die Güte von verschiedenen Ableitungen des Ekg zu prüfen. Das Prinzip beruht darauf, die Lage des Herzens eine Probanden durch Variation der Körperlage und der Atemlage maximal im Brustkorb zu verändern. Falls das Ableitefeld völlig parallelfädig verläuft, müßten sich die mit der Ableitemethode gewonnenen Vektorschleifen durch elektronische Rotation ineinander überführen lassen, da sich bei solcher Lageveränderung die kardialen Determinanten des Ekg sicher nicht wesentlich verändern. Die Verformungen der Konfiguration der Vektorschleife in Breitenund Kartenansicht sind ein Maß der Inhomogenitäten des Ableitefeldes, vorwiegend bedingt durch die Konfiguration des Brustraumes, die inhomogene Leitfähigkeit und die daraus resultierende mangelhafte Parallelität der Ableitelinien des Feldes. Es werden Formkriterien entwickelt, welche die Ähnlichkeit der Vektorschleifen, welche in verschiedenen Herzpositionen registriert werden, quantitativ ausdrücken. Diejenige Ableitung wird als die “beste” betrachtet, bei der die größte Ähnlichkeit der Schleifen bei extremer Dislokation und die kleinste Variation ihrer Raumwinkel besteht.
