Früh- und Spätergebnisse bei Hüftalloarthroplastiken

Zusammenfassung

Es wird über ein Material von 63 Hüftalloarthroplastiken berichtet. Von diesen konnten 38 nach einer Beobachtungszeit von 1–7 1/2 Jahren untersucht werden. Der alloplastische Kopf-Halsersatz bei Kopfnekrosen und Schenkelhalspseudarthrosen bei Patienten über 60 Jahren hat sich bewährt. Auf exakte Paßform der Prothese und auf zweckmäßige Behandlung häufiger calcipenischer Osteopathien ist zu achten. Bei Coxarthrosen haben die Alloplastiken meist enttäuscht, konnten aber bei doppelseitigem Leiden in Verbindung mit der Arthrodese der schlechteren Seite doch bisweilen langdauernd befriedigende Ergebnisse bringen. Bei schweren Dysplasiearthrosen sowie bei der Protrusio acetabuli ist von der Alloarthroplastik im allgemeinen abzuraten. Remobilisierung ankylotischer Hüften (nach Coxitiden oder rheumatischen Erkrankungen bzw. Bechterew) kann in Einzelfällen meist leider nur vorübergehende Erfolge bringen. Die Verwendung von Acryl-Spezialendoprothesen mit Stahlkern hat sich bewährt, übermäßiger Abrieb wurde nicht beobachtet. Auf die Wichtigkeit krankengymnastischer und hydrotherapeutischer Nachbehandlung wird hingewiesen.
