Summary

The indications and operative details for implantation of silicone-rubber protheses in hand surgery are discussed: The following operations are described: Temporary implantation of silastic-cable for two stage flexor tendonplasty, arthroplasty on MP and PIP joints also arthroplasty on wrist joint and replacement of carpal bones.

Zusammenfassung

Silicon-Kautschuk ermöglicht auf Grund seiner guten biologischen Verträglichkeit und günstigen mechanischen Eigenschaften die Anwendung auf verschiedenen Gebieten der Handchirurgie. Beim zweizeitigen Beugesehnenersatz kann ein Silastic-Kabel temporär als Platzhalter für die Sehne implantiert werden. Dabei kommt es um dieses Material herum zu einer fibroblastischen Reaktion des Sehnengleitlagers. Der so entstehende Bindegewebsschlauch dient später als Sehnenscheide, in die das Sehnentransplantat eingezogen wird.

Die von Swanson angegebenen Fingergelenke aus Silicon-Kautschuk lassen sich bei den verschiedenen Indikationen an PIP- und MP-Gelenken gut verwenden. Die Technik ist einfach. Komplikationen haben wir bisher nicht beobachtet.

Für die Therapie bei der Lunatummalacie scheint sich die Verwendung eines Blocks aus Silastic besonders anzubieten. Das Material wird entsprechend dem Op.-Situs in Form und Größe zurechtgeschnitzt.

Auch bei total zerstörten Handgelenken kann Silicon-Kautschuk als Interpositum verwendet werden. Dadurch lassen sich Schmerzfreiheit, Stabilität und Beweglichkeit wiedergewinnen.

Über das technische Vorgehen und eigene Erfahrungen mit den genannten Verfahren wird berichtet.
