Zusammenfassung
Eine progrediente, durch Ischämie, Hypoxie oder einen raumfordernden Prozeß ausgelöste Hirnschwellung kann zur druckbedingten Ischämie und damit zum Totalinfarkt des knöchern umgebenen Gehirns führen. Wird der Hirntod durch Reanimationsmaßnahmen überbrückt, kommt es zur „intravitalen“ Autolyse des Gehirns und — infolge der anhaltenden Schwellungsneigung — zur Verlagerung nekrotischen Kleinhirngewebes in den Subarachnoidalraum des Rückenmarkes. Gleichzeitig bilden sich in Randzonen zu noch normal durchbluteten Geweben demarkierende entzündliche Veränderungen aus, die in Hypophyse, oberem Halsmark sowie in Form einer Reaktion der spinalen weichen Häute auf die Kleinhirngewebstrümmer regelmäßig nachweisbar sind, wenn eine Manifestationszeit von 24–36 Std gegeben ist. Diese für das Verständnis des Hirntodes wesentlichen morphologischen Aspekte werden an Hand von 10 Beobachtungen dargelegt.
Angesichts der Latenz, mit der die Nekrophanerose dem Funktionsausfall des irreversibel geschädigten Gehirns folgt und der Dringlichkeit der Todeszeitbestimmung erscheint die Feststellung des Hirntodes nach klinischen Gesichtspunkten notwendig und auch möglich.
Summary
Cerebral swelling, induced by ischemia, hypoxia or expansive intracranial processes, can terminate in cerebral ischemia by tamponade and infarction of the hole brain encircled by bone. If this irreversible state is prolonged by artificial respiration and maintanance of the blood pressure, “intravital” autolysis of the brain and displacement of necrotic cerebellar tissue in the subarachnoid spinal space result. In the adjoined tissues of the dead brain, i.e. pituitary gland and upper cervical spinal cord, demarcating inflammatory reactions develop. Furtheron the displacement of cerebellar fragments is followed by an reaction of the pia and arachnoidea of the spinal cord.
Although these morphological aspects are essential, they verify the brain death only subsequently. Considering the urgent determination of the time of death it seems to be necessary and possible to diagnose the brain death corresponding to functional critereas.
