Does a gradually decreasing menstrual blood loss (MBL) in women wearing a Cu-IUD indicate a risk for an accidental pregnancy?

Abstract

In three separate series, comprising 71 women using either a Gravigard, a Nova-T or a Multiload contraceptive device, the menstrual blood loss (MBL) was quantitatively assessed before and after insertion of each respective IUD. In three women, an involuntary pregnancy was registered with the IUDin situ. In these three women a successively diminishing MBL was registered during the last 3 months before conception, while none of the remaining women included in the series experienced any accidental pregnancy or any reduction of the MBL. Thus, this report emphasizes the potential risk for an accidental pregnancy in women wearing a Cu-IUD if successively diminishing MBLs are observed.

Resumé

Dans trois séries distinctes portant sur 71 femmes qui utilisaient l'un des trois dispositifs contraceptifs Gravigard, Nova-T ou Multiload, on a évalué quantitativement les pertes de sang menstruelles avant et après l'insertion respective de chacun de ces DIU. Chez trois femmes, une grossesse involontaire s'est produite alors que le stérilet était en place. On a constaté chez ces trois femmes, pendant les trois derniers mois avant la conception, une diminution progressive des pertes de sang menstruelles, alors qu'aucune autre des femmes comprises dans ces séries n'était accidentellement tombée enceinte ou n'avait subi une réduction de ses règles. Ce rapport souligne donc le risque potentiel d'une grossesse accidentelle chez les femmes dotées d'un DIU au cuivre lorsque l'on observe une réduction progressive des pertes sanguines menstruelles.

Resumen

En tres series diferentes que comprendían 71 mujeres usando los dispositivos intrauterinos Gravigard, Nova-T o Multiload, el sangrado menstrual (menstrual blood loss—MBL) fue evaluado cuantitativamente antes y después de la inserción de los respectivos DIU. En tres mujeres se produjeron embarazos involuntarios con el DIU in situ. En estas tres mujeres se registró una disminución progresiva del sangrado menstrual durante los tres últimos meses antes de la concepción, mientras que ninguna de las mujeres restantes incluídas en las series experimentó un embarazo accidental o una disminución del sangrado menstrual. En consecuencia, se enfatiza el riesgo potencial de embarazo accidental en mujeres usando un DIU con cobre, si se observa una reducción sucesiva del sangrado menstrual.
