Possibilités thérapeutiques de l'hallux valgus

Résumé

Le traitement orthopédique à l'aide d'exercices musculaires ou d'orthèses n'a guère fait ses preuves. De multiples méthodes chirurgicales correctrices ont été proposées. Elles peuvent être classées en 4 groupes : 1. Les procédés agissant sur les parties molles qui comprennent la libération des rétractions capsulo-ligamentaires externes, associée à une technique de remise en tension des éléments internes : soit par le recentrage de la sangle sésamoïdienne décrite par Keller en 1904, Morton en 1935 et diffusé en France par Jean Lelièvre; soit par la technique de Petersen diffusée en France par Roy-Camille, consistant en une libération externe par voie commissurale avec sésamoïdectomie externe et exostosectomie interne ainsi qu'une suture intercapsulaire M1-M2; soit par l'opération de Mac Bride (1928), qui ajoute aux gestes précédents la transposition du tendon conjoint de l'abducteur oblique transverse détaché de la base de P1 et réimplanté sur le col de M1. 2. Les procédés de réaxation osseuse par ostéotomie : soit ostéotomie à la base du premier métatarsien corrigeant le métatarsus varus associé à un deuxième geste de recentrage de la sangle sésamoïdienne; soit ostéotomie à la base de la phalange proximale corrigeant l'hallux interphalangien et son effet d'arbre à came; soit ostéotomie double à la base de M1 et à la base de P1; soit une ostéotomie sous-capitale de M1 décrite par Hohmann en 1921, corrigeant en un seul site extra-articulaire toutes les déformations de l'hallux valgus. 3. Les procédés d'arthroplastie métatarsophalangienne associant au recentrage de la sangle sésamoïdienne un geste préalable articulaire : par résection de la base de P1 décrite par Keller en 1904 et Brandes en 1929. Parfois associée soit à une réimplantation ostéocartilagineuse de la base de P1 décrite par Regnaud, soit à une mise en place d'un implant en silastic de Swanson, technique décrite en 1973 par Michon, Delagoutte et Jandeaux. 4. L'arthrodèse métatarso-phalangienne du gros orteil est parfois indispensable dans les grosses déformations de l'hallux valgus ou dans les séquelles douloureuses de cure chirurgicale de l'hallux valgus.

Conservative treatment by means of muscle exercises or ortheses has not proved useful. Numerous methods for surgical correction have been suggested, and may be classified in 4 groups: 1. Procedures on the soft parts, comprising relaxation of the lateral capsular and ligamentous contractions combined with a technique for tightening the medial structures; a) by recentering of the sesamoid sling as described by Keller in 1904 and Morton in 1935 and popularised in France by Jean Lelièvre; by Petersen's technique, advocated in France by Roy-Camille, consisting of a lateral freeing by a commissural approach with lateral sesamoidectomy and medial resection of the exostosis, together with intercapsular suture of M1-M2; c) by MacBride's operation (1928); which adds to the foregoing procedures transposition of the conjoint tendon of the oblique and transverse heads of the adductor hallucis, detached from the base of P1 and reinserted at the neck of M1. 2. Procedures for bony realignment by osteotomy: a) by osteotomy of the base of the first metatarsal to correct the metatarsus varus, combined with a second procedure for recentering the sesamoid sling; b) by osteotomy of the base of the proximal phalanx to correct the interphalangeal valgus and its camshaft effect; c) by a double osteotomy of the base of M1 and the base of P1; d) by the subcapital osteotomy of M1 described by Hohmann in 1921, correcting all the deformities of hallux valgus at a single extra-articular site. 3. Procedures for metatarsophalangeal arthroplasty combining a preliminary articular procedures with recentering of the sesamoid sling: a) by resection of the base of P1 as described by Keller in 1904 and by Brandes in 1929; b) sometimes combined either with osteocartilaginous reimplantation of the base of P1, as described by Regnauld, or with the insertion of a Swanson silastic implant according to the technique described in 1973 by Michon, Delagoutte and Jandeaux. 4. Metatarso-phalangeal arthrodesis of the great toe is sometimes necessary for major deformation in hallux valgus, or for painful sequelae of the operative treatment of this condition.
