247. Der Arteria radialis-Lappen — ein Ersatz der gestielten Fernlappenplastik zur Hand?

Soft tissue defects of the hand demand early coverage. In comparison to the distant flap, the forearm flap seems to have three advantages: one-step surgical procedure, early mobilization of the hand, and a short period of hospitalization. The forearm flap can be used as an inverted flap or as a free flap. It can be used for primary or early secondary reconstruction. The preferred sites for its usage are the dorsal and palmar side of the hand, as well as the first interdigital fold. Contraindications are enlarged defects for both techniques and traumatic interruptions of the superficial palmar arterial arch for the inverted flap alone.

Weichteildefekte der Hand bedürfen einer schnellen Deckung. Gegenüber den gestielten Femlappenplastiken hat der Arteria radialis Lappen 3 Vorteile: einzeitige Operation, frühe Mobilisation, kurzer stationärer Aufenthalt. Er kann als Umkehrlappen oder freier Lappen primär oder frühsekundär eingesetzt werden. Bevorzugte Anwendungsgebiete sind Handrücken, Daumen und erste Zwischenfingerfalte. Als Kontraindikation gelten fir beide Techniken übergroße Hautdefekte und für den Umkehrlappen Lasionen des Arcus palmaris superficialis.
