38. Rekonstruktion der Speiseröhre bei Atresien

Summary

Up to the present date 649 cases of atresia of the oesophagus have been treated at the pediatric surgical hospital at Bremen. In the majority of the cases direct anastomosis end to end without myotomia and without plastic reconstruction of the wall was possible. If the distance between the upper and the lower pouch of the oesophagus was too long, auxiliary steps had to be taken, for example interposition of allo- or xenografts. For the last ten years the pediatric surgeons at Bremen dispensed with this kind of operation and preferred the elongation of the oesophagus which is described by Howard resp. by Hays or Rehbein.

Zusammenfassung

An der Kinderchirurgischen Klinik in Bremen wurden bis heute 649 Oesophagusatresien behandelt. In der Mehrzahl der Falle gelang eine Direktanastomose End-zu-End ohne Myotomie und ohne Wandplastik. War der Abstand zwischen oberem und unterem Speiser6hrenblindsack zu groß, mul3te zu Hilfsmaßnahmen, zur Interposition von Eigen- oder Fremdmaterial gegriffen werden. Seit 10 Jahren haben die Bremer Kinderchirurgen auf diese Operation verzichtet und die Elongationsverfahren nach Howard bzw. nach Hays oder Rehbein favorisiert.
