A case of intractable pain in the head and face associated with pathological changes in the optic thalamus

Summary

This case concerns a woman who had intractable pain in the left side of the head and face. The pain was not relieved by a complete trigeminal root section.

Later, at post mortem, it was found that there were bilateral lesions within the optic thalami affecting chiefly the centrum medianum and part of the postero-lateral ventral nucleus.

This case calls attention to the possible necessity of having to make bilateral optic thalamic lesions, even for the relief of unilateral pains.

Zusammenfassung

Der beschriebene Fall betrifft eine Frau, die unter unbeeinflußbaren Schmerzen der linken Kopf- und Gesichtsseite litt. Der Schmerz konnte auch durch vollständige Durchschneidung der Trigeminuswurzel nicht beseitigt werden.

Postmortal fanden sich bilaterale Läsionen im Thalamus, die vorwiegend das Centrum medianum und einen Teil des Nucleus posterolateralis ventralis betrafen.

Der Fall weist darauf hin, daß es möglicherweise notwendig ist, auch bei einseitigen Schmerzzuständen doppelseitige Läsionen im Thalamus zu setzen.

Résumé

Ce cas concerne une femme qui avait une douleur incurable faciale gauche. La douleur ne guérit pas après section de la racine trigéminale. Plus tard après la mort on trouva qu'il y avait des lésions bilatérales du thalamus affectant principalement le centre et une partie du noyau postéro-latéral ventral.

Ce cas attire l'attention sur la nécessaté qu'il a peut-être de réaliser des lésions thalamiques bilatérales même pour guérir des douleurs unilatérales.

Riassunto

Viene descritto il caso di una donna affetta da dolore intrattabile al lato sinistro del cranio e delia faccia. Il dolore non venne influenzato da una sezione completa delia radice trigeminale.

All'esame autopsico furono riscontrate lesioni bilaterali del talamo interessanti principalmente il centro mediano ed il nucleo postero-Iaterale ventrale.

Questo caso richiama l'attenzione sulla possibilità di avère a che fare anche negli stati dolorosi unilaterali con lesioni talamiche bilaterali.

Resumen

El caso descrito concierne a una mujer que sufría dolores rebeldes de la mitad izquierda de la cabeza y cara. La sección completa de la raiz del trigémino tampoco consiguió eliminar el dolor.

En la autopsia se encontraron lesiones bilaterales en el tálamo que afectaban fundamentalmente el centro mediano y una parte del nucleo ventral posterior y lateral.

El caso demuestra que posiblemente son necesarias lesiones bilaterales del tálamo aun en los estados dolorosos unilaterales.
