Die Prüfung der Hemisphärendominanz und Händigkeit in der Neurochirurgie

Zusammenfassung

In der Arbeit wird über das Ergebnis von 2 Untersuchungsreihen berichtet:

• 1. An einem Probandengut von 215 Normalpersonen wurde auf der Grundlage eines ausführlichen Fragebogens und durch Vergleich verschiedener Tests versucht, eine aussagekräftige in kurzer Zeit durchführbare Methode zur praktischen Bestimmung von Händigkeit und Hemisphären-dominanz in der Neurochirurgie zu finden. Sie besteht im Händeklatschtest, einem Zeichentest und einem neuartigen Geschicklichkeitstest. Nachprüfung an großem Krankengut und Beachtung der Dominanzfrage durch alle Neurochirurgen wird empfohlen, da sie nicht nur für die Neurochirurgie von Wichtigkeit ist, sondern auch zur Klärung offener Fragen auf diesem Gebiet beitragen kann.

• 2. Ein neurochirurgisches Krankengut von 193 Patienten mit einseitigen Großhirngeschwülsten wurde in bezug auf Händigkeit und Hemisphären-dominanz untersucht. Die Ergebnisse entsprachen der Erwartung, daß im allgemeinen bei Rechtshändern die linke Hemisphäre, bei Linkshändern die rechte Hemisphäre die dominante ist, jedoch sind Ausnahmen und die nicht ganz seltene Ambivalenz der Hemisphären zu beachten. Die Sicherheit der Untersuchungsergebnisse in dieser Reihe war durch die mangelhaften Händigkeitsprüfungen der Patienten beeinträchtigt, ist aber auch teilweise durch ihren Zustand vermindert, der eingehende Tests nicht erlaubt. Daher wird vorgeschlagen, gelegentlich von vorsorgenden Untersuchungen, auch mit kurzen Tests, die Händigkeit bei Normalpersonen zu prüfen und zu registrieren.

Summary

In this paper the results of two series of investigations are described:

• 1. As a control series 215 normal people were investigated on the basis of a detailed questionnaire and by comparing different tests in order to discover a thoroughly reliable method, which could be quickly performed, as a practical means of identifying handedness and hemisphere dominance in neurosurgery. The tests consisted of a hand-clapping test, a pointing test, and a new type of dexterity test. The authors recommend verification in a large series of patients and attention to the questions of dominance by all neurosurgeons, because this is of importance not only for the neurosurgeon but it can also help in the clarification of the open questions in this field.

• 2. A series of 193 neurosurgical patients with unilateral tumours of the cerebral hemisphere were studied in relation to handedness and hemisphere dominance. The results corresponded to those expected, in that generally the dominant hemisphere is the left in the right-handed, and is the right in the left-handed. However, there are exceptions and attention should be paid to ambivalence of the hemispheres which is not very rare. The reliability of the results of the investigations in these series was impaired by the incomplete identification of handedness in the patients, but it is also diminished partly through their state which did not allow thorough tests. Because of this it is proposed that when an occasion for careful investigations arises the handedness of normal persons should be identified, by means of short tests, and recorded.
