Zusammenfassung
5 Fälle mit Strahlenspätnekrosen des Hirnstammes einschließlich des Hypothalamus nach Bestrahlung mit ultraharten Röntgenstrahlen und schnellen Elektronen (Betatron) wurden mitgeteilt, davon in 4 Fällen die neuropathologischen Befunde. Die Symptomatik der Strahlenenzephalopathie setzte nach einem freien Intervall von 4 bis 30 Monaten ein. Die Krankheitsdauer betrug 3 Wochen bis 35 Monate. Die autoptischen Befunde einer akuten Koronarthrombose, einer Lungenembolie, eines Herzversagens bei Lungenödem müssen als sekundäre Folgen des schweren strahleninduzierten zerebralen Syndroms angesehen werden. Die Lokalisation der Strahlenspätnekrosen entspricht der Dosisverteilungskurve. Das typische Fortschreiten der geweblichen Veränderungen von zentral nach peripher ist mit der Dosis-Latenzzeit-Relation zu erklären. Eine besondere gewebliche Strahlenempfindlichkeit des Hirnstammes scheint nur durch den Reichtum an myelinhaltigen Fasern und durch die selektive Radiovulnerabilität der Markscheidenglia gegeben zu sein. Eine spezifische Strahlenempfindlichkeit der Hypothalamuskerne scheint nicht vorzuliegen. Im wesentlichen dürfte die geringere Strahlentoleranz des Hirnstammes auf anatomische Tatsachen zurückzuführen sein: Einerseits ist der Entwicklung von Nekrosen im Hirnstamm durch seinen kleinen Querschnitt, andererseits durch die lebenswichtige Funktion der hypothalamischen, mesenzephalen und bulbären Kerne eine frühe Grenze gesetzt. Eine gezielte Bestrahlung eng umschriebener intrakranieller, besonders hirnstammnaher oder im Hirnstamm selbst gelegener Strukturen kann ohne Mitschädigung benachbarter Bezirke mit der externen Strahlentherapie kaum erreicht werden. Eine Zweitbestrahlung, auch nach vielen Jahren, kann über einen Summationseffekt zu einer manifesten Strahlenenzephalopathie führen.
Summary
Five patients with late radiation necrosis of the brain stem, including the hypothalamus, following radiation with ultra hard X-rays and high speed electrons (Betatron) are discussed together with the neuro-pathalogical findings in 4 of them. After a trouble-free period of between—4–30 months, the symptoms of radiation encephalopathy developed. The radiation-induced illness lasted from 3 weeks to 35 months. The autopsy findings, comprising 1 acute coronary thrombosis, 1 pulmonary embolism, 1 heart failure due to pulmonary oedema, must be considered to be the consequences of a severe radiation induced cerebral syndrome. The localization of the radiation necrosis corresponded to the dose distribution curve. The typical progress of the tissue changes from central to peripheral is to be explained on the basis of a dose-latency relationship. It seems that a special tissue sensitivity of the brain-stem derives from the richness of myelinated fibres and the selective radio-vulnerability of the glia of the myelin sheaths (interfascicular glia). There seems to be no evidence of a specific radiation sensitivity of the hypothalamic nuclei. For the main part the low radiation tolerance of the brain stem can be attributed to anatomical factors: firstly, it is caused by the development of necrosis in the brain stem due to its small cross-section, and secondly, by the low threshold resulting from the fundamental importance of the functions contained in the hypothalamic, the mesencephalic, and the bulbar nuclei. Using external radiation it is scarcely possible to achieve a precisely aimed radiation of the narrowly circumscribed intracranial structures, especially those which lie in the vicinity of the brain stem or in the brain stem itself, without simultaneously damaging neighbouring regions of the brain. A second course of treatment, even many years later, can lead to an obvious radiation encephalopathy as a result of summation.
