Lymph drainage of the superior lobe of the left lung

Summary

The author presents the results of a study of the lymph drainage of the superior lobe of the left lung in 50 post mortem subjects. The lymphatics were injected using a modification of Gerota's method.

The lymphatics emerge from this lobe of the lung as three distinct channels each of which is divided into subchannels. The lymph channels running towards the homo- or controlateral mediastinum are described in detail. The lymph nodes located along these drainage pathways are also described.

This study allowed to identify little known or unknown lymph node systems, e.g. the left inominate channel with three subducts, the lymph channel of the aortic arch, the major and minor azygoaortic channels.

Finally, the results of this study allowed to demonstrate the drainage of lymph directly towards the thoracic duct and indirectly towards the subclavian, infrahyoid and carotid regions.

Résumé

L'auteur présente les résultats obtenus après étude des lymphatiques du lobe pulmonaire supérieur gauche sur 50 sujets. Le colorant utilisé a été la masse de Gérota modifiée par une technique personnelle.

Les lymphatiques émergent de ce lobe regroupés en trois pédicules parfaitement individualisés eux mêmes sub-divisés en sous pédicules. Les voies lymphatiques qui se dirigent vers le médiastin soit homolatéralement, soit controlatéralement sont décrites en détail. Les centres ganglionnaires récepteurs de ce drainage lymphatique so également précisés.

Ce travail permet de rappeler l'existence de chaînes ganglionnaires peu connues, voire non encore décrites dans les traités classiques: chaîne innominée gauche avec trois sous-chaînes, chaîne de la crosse aortique, chaîne azygo-aortique maieure et azygo-aortique mineure etc.

Ce travail a également permis de mettre en évidence un drainage lymphatique direct vers le canal thoracique et des drainages lymphatiques indirects vers les régions sus-claviculaire, infra-hyoïdienne et carotidienne.
