Résumé

L'ostéotomie de flexion céphalique décrite par P. Teinturier a été réalisée sur six hanches chez cinq patients. Les résultats antalgiques à court terme ont été très satisfaisants mais peu durables. Quatre hanches ont été reprises par prothèse totale dans un délai inférieur à trois ans. Deux patients bénéficient d'un bon résultat après cinq ans. Les indications de ce type d'ostéotomie doivent rester limitées et pour notre part nous avons abandonné l'ostéotomie de flexion pure, mais nous l'associons volontiers aux ostéotomies de varisation et de translation interne, lorsque le bilan radiologique de recentrage confirme la correction d'une subluxation antérieure par la flexion céphalique.

Summary

Five patients (six hips) underwent an intertrochanteric osteotomy stabilized with a 30° flexion of the proximal part of the femur. This procedure, described by P. Teinturier, combines the biological effect of the osteotomy with the treatment of anterior subluxation of the femoral head allowing wider contact between femoral and acetabular cartilages to stabilize the arthrotic process. Short term results were encouraging except for one pseudarthrosis of the osteotomy. The initial excellent pain relief did not last in four cases who needed reoperation and total hip replacement with problems due to the morphological changes of the upper end of the femur. Two patients remain painless and have a mobile and stable hip five years after operation. We do not still use this isolated flexion osteotomy, but flexion of the femoral head associated with varisation or McMurray osteotomy is still to be considered when the hip shows an anterior subluxation and when the clinical and radiological assessment of the case undoubtedly justifies the osteotomy.
