Zusammenfassung
In eigenen früheren Untersuchungen konnten wir bei gesunden Versuchspersonen eine physiologische Tagesrhythmik der ACTH-Sekretion dann nachweisen, wenn die Patienten unter Ruhebedingungen im Bett gehalten wurden. Sie war in gleicher Weise bei jedem erniedrigten Steroidspiegel im Blut unabhängig vom aktuellen ACTH-Spiegel, z. B. auch beim subtotal adrenalektomierten Cushing-Kranken mit exzessiv erhöhten ACTH-Blutspiegeln, einwandfrei zu erkennen. Dagegen war diese Tagesrhythmik bei therapeutischen und spontan erhöhten Steroidspiegeln im Blut immer aufgehoben.
Aufgrund dieser Befunde möchten wir die folgenden Vorstellungen über den Regulationsmechanismus von ACTH diskutieren:
Im Hypothalamus nehmen wir zwei autonome Regulationszentren an, die für die Sekretion von ACTH verantwortlich sind. Beide werden durch Steroide gebremst. Das eine für die Bildung von CRF verantwortliche Zentrum reagiert sofort auf jede Änderung im peripheren Steroidblutspiegel und reguliert die aktuelle ACTH-Sekretion. Das andere für die Tagesrhythmik verantwortliche Zentrum reagiert nur langsam und versucht, bei allen Ruhe- und Unterfunktionszuständen der NNR auf tonische Art die Sekretion von ACTH und Cortisol anzuregen und zu erhalten.
Bei spontanen oder therapeutischen Erhöhungen des peripheren Steroidblutspiegels ist dieses Zentrum jedoch völlig blockiert und die Rhythmik nicht mehr nachweisbar.
Abstract
In a recently published paper we were able to demonstrate a reproducable diurnal rhythm in ACTH and cortisol secretion in metabolically normal persons only if these patients were kept under resting conditions in the hospital. This diurnal variation was also present in all cases with decreased blood steroid levels regardless of blood ACTH levels, e. g. in subtotal adrenalectomized Cushing's patients. The rhythm, however, was absent in all cases with elevated blood steroid levels induced either spontaneously or therapeutically.
From these results we would like to propose the following concept: There may exist two autonomous hypothalamic receptor sites controlling the release of ACTH. Both are responsive to the feedback of corticosteroids. In the case of CRF receptors, steroids are quickly active and regulate rapid responses. In the case of the center controlling rhythmic secretion of ACTH, responses are slower, and regulate in a more tonic fashion the secretion of ACTH. Under all conditions where blood steroids are high (exogenous administration and Cushing's syndrome), however, this special hypothalamic center is blocked or inactive.
