Zusammenfassung
Das gehäufte Auftreten von Coxsackie B5-Erkrankungen in der Bundesrepublik während der Sommer- und Herbstmonate des Jahres 1962 gab Gelegenheit, den diagnostischen Wert der Coxsackie B5-KBR an einem größeren Patientenkollektiv zu überprüfen. Dabei wurden mit Frigen gereinigte Gewebekulturantigene (Fl-Zellen) verwendet, die nach ultrazentrifugaler Anreicherung einen typenspezifischen Titer von —log 1,8–2,4 aufwiesen (Mikromethode, Plattentest, Kolmertechnik). Die optimale Antigenverdünnung für die Durchführung der Routineuntersuchungen wurde in der Isofixationstechnik nachAlmeida ermittelt. Die Antigene reagierten nicht mit komplementbindenden (kb.) Immunseren gegen die übrigen Vertreter der Coxsackie B-Gruppe und ließen auch mit kb. Antiseren gegen das Coxsackie A9-Virus und die drei Typen des Poliovirus keine Reaktion in der KBR erkennen. Die typenspezifischen kb. Hyperimmunseren wurden an Meerschweinchen hergestellt. Der homologe kb. Antikörpertiter dieser Seren betrug bei Verwendung der optimalen Antigendosis — log 2,8–3,1. Die Untersuchung der Seren von 92 Patienten, bei denen aus Stuhl, Liquor oder Rachenabstrich Coxsackie B5-Virus isoliert werden konnte, erbrachte in 60 Fällen (=65,3%) ein positives Ergebnis in der KBR und verlief in 32 Fällen (=34,7%) negativ. In dieser letzteren Zahl sind jedoch auch diejenigen Fälle enthalten, von denen lediglich Seren aus den ersten beiden Wochen nach Auftreten der Allgemeinsymptome zur Untersuchung gelangten. Mithin verlief die KBR bei 13 Patienten mit positiver Virusisolierung negativ, von denen uns Seren aus der ersten sowie nach der zweiten bis dritten Krankheitswoche zur Verfügung standen. Die gleichzeitig durchgeführte Testung der Seren von diesen 92 Patienten auf kb. Antikörper gegen die drei Typen des Poliovirus ließ in 12 Fällen (=20% der Seren, die kb. Antikörper gegen Coxsackie B5-Virus enthielten) eine schwache Reaktion gegen Typ III des Poliovirus erkennen. Die kreuzweise KBR mit Coxsackie B5- und Poliovirus-Typ III-Antigenen und den entsprechenden kb. Meerschweinchenhyperimmunseren ergab keine Hinweise für das Vorliegen einer antigenen Beziehung dieser beiden Vertreter der Enterovirusgruppe. — Bei Verwendung geeigneter Antigene und einer entsprechenden Versuchsmethodik stellt die Coxsackie B5-KBR eine Ergänzung der Möglichkeiten in der Enterovirus-Diagnostik dar, die auch bei negativem virologischem Befund durch Titerdifferenz in Doppelseren unter Umständen die Diagnosestellung ermöglicht.
Summary
The mass appearance of Coxsackie B5 illnesses in the Federal Republic of Germany during the summer and autumn of 1962 provided an opportunity for examining the diagnostic value of Coxsackie B5 complement fixation test in a greater number of patients. We used fluorocarbon-purified tissue culture antigens (Fl-cells) which after concentration in the ultra-centrifuge showed a type-specific titer of —log 1,8 to 2,4 (micro method, perspex plate test, Kolmer technique). The optimal antigen dilution for routine tests was assessed by means of the isofixation technique according toAlmeida. The antigens did not react with complement-fixing immune sera against the other types of Coxsackie B group as well as against Coxsackie A9 virus and the three types of poliomyelitis virus. The type-specific hyperimmune sera were produced in guinea pigs. The homologous complement-fixing antibody titer of these sera, when using the optimal antigen dosage, was —log 2,8 to 3,1. Examination of the sera of 92 patients with positive Coxsackie B5 virus isolation in feces, liquor or throat washings gave in 60 cases (=65,3%) a positive result in complement fixation and showed negative findings in 32 cases (=34,7%). This latter number contains also those cases of which only sera of the first two weeks after onset of symptoms were examined. Thus the complement fixation test showed negative results only in 13 patients with positive virus isolation whose sera were available to us from the first as well as the second to third week of illness. Simultaneous examinations of the sera of these 92 patients for complement-fixing antibodies against the three types of the poliovirus demonstrated in 12 cases (=20% of the sera which contained complement-fixing antibodies against Coxsackie B5 virus) a weak reaction to type III of poliovirus. The cross-wise complement fixation test with Coxsackie B5 and poliovirus type III antigens and the corresponding complement-fixing guinea pig hyper-immune sera gave no indications of an antigenic relationship of these two members of the enterovirus group. When applying suitable antigens and an adequate test method, the Coxsackie B5 complement fixation test offers a supplement to the possibilities in enterovirus diagnostic which also in the case of negative virological findings might make a diagnosis possible.
