Summary
We reviewed 223 idiopathic scoliosis, juvenile scoliosis (71%) or adolescent scoliosis (29%), treated by Wilmington Body Jacket at an average age of 13 years. The mean angular value when treatment started was 26° for the double-major curves, 28° for the single thoracic curves, 23° for the lumbothoracic curves, 24° for the lumbar scoliosis. There are 50% angular corrections within the body jacket. At the end of the treatment, the average age is 15 years 5 months (the average duration of treatment is 2 years 2 months). At all, 53% of curves are stabilized (± 5°), 31,5% improved 6° or more, and 15,5% progressed more than 6° (but only 12 scoliosis (7%) warrant fusion. 8 of them were thoracic curves). With regards to the thoracolumbar and the lumbar curves, a stabilization (± 5°) or an improvement more than 6° are obtained in 83 to 90% of cases, depending on whether the initial angles were more or less than 30°. The topography of the curves treated (49% of lumbar curves, 14% of thoracolumbar curves, 26% of thoracic curves, with apex level below T7, 11% of double major curves) points out the qualities of correction and the efficacity of these orthosis at a lumbar and thoracolumbar level and it shows our care to maintain these curves below 30° at bone maturity.
Résumé
Nous avons revu 223 scolioses idiopathiques, scolioses juvéniles (71 %) ou de l'adolescent (29 %), traitées par Body-Jacket de Wilmington, à un âge moyen de 13 ans. La valeur angulaire moyenne à la mise en place du traitement était de 26° pour les doubles majeures, 28° pour les courbures thoraciques uniques, 23° pour les thoraco-lombaires, 24° pour les lombaires. Les corrections angulaires sous corset sont de l'ordre de 50 %. A l'arrêt du traitement, (âge moyen de 15 ans 5 mois, durée moyenne du traitement 2 ans 2 mois) on constate globalement une stabilisation angulaire (+ 5°) dans 53 % des cas, une amélioration supérieure à 6° dans 31 % des cas, une aggravation supérieure à 6° dans 15 % des cas. 12 scolioses (7 %) (dont 8 de localisation thoracique) ont été opérées. Concernant les courbes thoraco-lombaires et lombaires, une stabilisation (± 5°) ou une amélioration sont obtenues dans 83 à 99 % des cas, selon qu'il s'agit d'angles inférieurs ou supérieurs à 30°. La répartition topographique des courbes traitées (49 % lombaires, 14 % thoraco-lombaires, 26 % thoraciques de sommet inférieur à D7, 11 % doubles majeures) traduit les qualités de réduction et de maintien de cette orthèse au niveau lombaire et thoracolombaire, et notre souci de maintenir ces courbes en deçà de 30° à maturation osseuse.
