Infective endocarditis of an interventricular patch caused byAcinetobacter haemolyticus

Summary

A case of infective endocarditis caused byAcinetobacter haemolyticus affecting an interventricular patch is reported. The patient, a 21-year-old man with a Fallot's tetralogy who had undergone cardiovascular surgery several years before, received imipenem and gentamicin for 6 and 4 weeks respectively and showed a good response without needing surgical treatment. Endocarditis byAcinetobacter species is very unusual and, to our knowledge, this is the first reported case of infective endocarditis caused byA. haemolyticus. As the clinical characteristics and the response to antibiotics appear to be similar to those reported for infective endocarditis byAcinetobacter lwoffi, prosthetic infective endocarditis byA. haemolyticus is apparently not always an indication for surgical treatment.

Zusammenfassung

Wir berichten über einen Fall von Interventrikulär-Patch-Endokarditis durchAcinetobacter haemolyticus. Betroffen war ein 21-jähriger Mann, bei dem vor einigen Jahren eine operative Korrektur einer Fallot'schen Tetralogie vorgenommen worden war. Die Behandlung erfolgte mit Imipenem und Gentamicin für jeweils 4 und 6 Wochen. Das Ansprechen war gut, ein chirurgischer Eingriff war nicht erforderlich. Unseres Wissens ist die Endokarditis durchAcinetobacter spp. sehr ungewöhnlich, und dies ist der erste Fall einer durchA. haemolyticus verursachten Endokarditis. Die klinischen Charakteristika und das Ansprechen auf Antibiotika scheinen ähnlich zu sein wie bei Fällen von Endokarditis durchAcinetobacter lwoffii. Offensichtlich muß bei einner Prothesenendokarditis durchA. haemolyticus nicht immer eine operative Behandlung durchgeführt werden.
