Zusammenfassung

Es wird eine Übersicht gegeben über die internationalen Erfahrungen mit Cephacetril hinsichtlich Bakteriologie, Pharmakologie und Klinik. Cephacetril hat dasselbe antibakterielle Wirkungsspektrum wie die anderen eingeführten Cephalosporine. Im Gegensatz zu einigen dieser Präparate besitzt Cephacetril eine recht gute Stabilität gengenüber einigen β-Lactamasen. Cephacetril weist eine geringe Eiweißbindung auf und bietet einen hohen Anteil von freiem Wirkstoff für Verteilung und antibakteriellen Effekt. Das apparente Verteilungsvolumen ist hoch im Vergleich zu den meisten anderen Cephalosporinen. Die Verträglichkeit, insbesondere die Nierenverträglichkeit des Präparates, ist gut, was u. a. in der Höhe der maximalen Tagesdosis zum Ausdruck kommt sowie im Fehlen von klinischen Berichten über Nephrotoxizität. Cephacetril kann sowohl intravenös als auch intramuskulär verabreicht werden. Die Indikationen für Cephacetril sind dieselben wie für die übrigen eingeführten Cephalosporine, jedoch besitzt Cephacetril als zusätzliche Indikation die bakterielle Meningitis, verursacht durch empfindliche Erreger, bei der es eine hohe therapeutische Erfolgsquote aufweist.

Summary

The paper contains a review of the international experience obtained with cephacetrile as regards the bacteriological, pharmacological and clinical characteristics of the drug. Cephacetrile has the same spectrum of antibacterial action as the other introduced cephalosporins. Unlike some of the latter, cephacetrile is markedly stable with respect to various β-lactamases. Cephacetrile demonstrates low protein binding, which means that a large portion of the drug is distributed in free form and thus has a direct antibacterial effect. The distribution volume is high in comparison with that of most other cephalosporins. The tolerability of the drug, including in particular its renal tolerability, is good, as is evident from the size of the maximum permissible daily dose and the absence of any clinical reports on nephrotoxicity. Cephacetrile can be given both i. v. and i. m. The indications for cephacetrile are the same as those for other cephalosporins, with the exception that it can also be used to very good effect in cases of bacterial meningitis provided the pathogens are sensitive to the drug.
