Zusammenfassung
Furosemid und Acetazolamid beeinflussen die renale Phenolrotausscheidung gegensätzlich. In der gewählten Versuchsanordnung steigert die i. v. Gabe von 20 bzw. 40 mg Furosemid den 15-min-Harnfluß im Mittel um 189,6 bzw. 345,9 ml, während die PSP-Elimination im gleichen Zeitraum um durchschnittlich 1,93 bzw. 8,32% abnimmt. Nach 250 mg Acetazolamid i.v. erhöht sich der 15-min-Harnfluß im Mittel um 16,9 ml, während die PSP-Ausscheidung durchschnittlich um 2,94% ansteigt.
Die Ursachen für die differente Beeinflussung der Farbstoffelimination durch die beiden Diuretika werden diskutiert. Es läßt sich wahrscheinlich machen, daß die Dissoziation der renalen PSP-Exkretion bei nur quantitativ verschiedener diuretischer Wirkung in erster Linie auf der unterschiedlichen Richtung der non ionic diffusion infolge differenter Beeinflussung des pH der Tubulusflüssigkeit beruht.
Summary
The renal excretion of phenolred (PSP) as a weak organic acid is accomplished in the proximal tubule by an energy-requiring system susceptible to metabolic inhibition. Otherwise compounds of this classe undergo subsequent tubular reabsorption partially practicable on the basis of physical properties and physiological variables.
Our investigation deals with the fact, that the different acting diuretics Furosemid and Acetazolamid influence the renal excretion of phenolred in opposite ways.
The studies were performed on 70 patients with normal and reduced renal function. 42 probands received 20 mg of Furosemid (group 1), 13 got 40 mg Furosemid (group 2) and 10 were given 250 mg Acetazolamid (group 3). The urine excretion volumes and PSP excretion after application of the diuretics, were related to the values found in the simple PSP test.
In the chosen experiment we observed the following mean changes: The i.v. injection of 20 and 40 mg resp. of Furosemid increased the 15 min urine flow by 189,6 and 345,9 ml resp. and lowered the pH of the urine by 0,35 (range 5,4 to 6,5) whilst the PSP elimination decreased by 1,93 and 8,32% resp. in the same period.
After the i.v. injection of 250 mg of Acetazolamid the mean urine flow increased by 16,9 ml, the pH elevation reached 2,57 (range 7,26–8,02) and PSP excretion increased by 2,94%.
The reasons for the different influence on the dye elimination by these two diuretics are discussed. It is suggested that the dissociation of the renal PSP elimination in only quantitatively different diuretic effect is caused in the first instance by the different direction of the non-ionic diffusion as a result of the influence on the pH of the tubular fluid.
