Un certain nombre d'indications peuvent être dégagées de la table ronde. Pour les Burst thoraciques, l'ostéosynthèse peut être postérieure, large et associée à une greffe. Au niveau dorso-lombaire, la synthèse peut être uniquement postérieure si elle est longue et associée à une greffe. Si la fonction est entravée par une greffe lombaire extensive, il faut discuter un abord antérieur pour éviter une raideur future. Pour les Burst lombaires, il faut utiliser un montage postérieur court. Un abord antérieur complémentaire est recommandé pour une greffe intersomatique à un niveau. L'ablation du matériel postérieur se fera à un an. Pour les fractures compression, la synthèse postérieure suffit à tous les niveaux. Le traitement orthopédique est décevant. Pour les seat-belt et les dislocations, la synthèse postérieure semble suffisante, mais nos cas ne sont pas assez nombreux pour être affirmatif.
Summary
Some indications can be extracted from this round table. Thoracic instrumentation can be posterior and extensive, associated with a graft in Burst fractures. At the thoracolumbar junction posterior instrumentation is sufficient when long enough with a supplementary graft. As an extensive lumbar graft may limit function, an anterior approach should be considered to avoid future stiffness. For lumbar Burst fractures short posterior instrumentation should be used. A complementary anterior approach is recommended for a single level interbody fusion. The posterior metalwork can be removed after one year. Posterior instrumentation is adequate for compression fractures at all levels. Orthopaedic treatment is disappointing for these cases. For seatbelt and dislocation fractures, posterior instrumentation seems to be sufficient, but we have too few cases to confirm this hypothesis.
