Résumé
Les ostéosynthèses du rachis par voie transpédiculaire répondent à des impératifs précis. La voie postérieure permet, dans la majorité des cas, la réduction de la déformation, la fixation permet de pérenniser cette réduction et autorise une décompression large de bonne qualité. Se discutent les problèmes de montages longs et de montages courts. Le montage long permet de compléter la réduction par le bras de levier que donne l'instrumentation lors de sa mise en place. Au contraire du montage court qui ne fait que pérenniser la réduction obtenue lors de l'installation du patient en décubitus ventral. L'un et l'autre ont donc chacun leurs indications qui sont soigneusement détaillées. L'indication d'une greffe complémentaire n'est justifiée qu'au niveau de l'étage pathologique mais certainement pas aux autres étages. Un abord antérieur complémentaire est justifié en cas de persistance d'une compression antérieure ou en cas de défects osseux importants, source d'instabilité secondaire.
Summary
Spinal osteosynthesis through a transpedicular approach has precise requirements. The posterior approach usually allows reduction of the deformity with the fixation maintaining the reduction and allowing a wide decompression of good quality. The problems of long and short assemblies are a matter of debate. Long assemblies allow completion of the reduction by the level arm of the instrumentation during its placement. On the other hand, the short assembly only maintains the reduction obtained during the positioning of the patient in the prone position. There are, therefore, separate indications for each which are carefully detailed. Additional grafting is only justified for the pathological level fracture. A complementary anterior approach is justified for persistent anterior compression, or for major osseous defects, which are a source of secondary instability.
