Zusammenfassung
1. Durch konstante Infusion von Palmitinsäure-9,10-H3 in Form eines Albuminkomplexes werden Umsatzraten der freien Fettsäuren und Triglyceride, Triglycerid-Umsatzzeit und Triglycerid-Clearance bei Patienten mit Hyperthyreose bestimmt und mit stoffwechselgesunden Personen verglichen.
2. Die Konzentration der freien Fettsäuren (p<0,001) und die Umsatzrate der freien Fettsäuren (p<0,001) ist bei Patienten mit Hyperthyreose etwa auf das Doppelte erhöht. Für Stoffwechselgesunde und für Patienten mit Schilddrüsenüberfunktion besteht eine positive Korrelation zwischen Konzentration an freien Fettsäuren und Umsatzrate der freien Fettsäuren im Plasma (p<0,001). Die „fractional turnover rate“ ist nur gering erhöht (p<0,05).
3. Trotz der hohen Konzentration der freien Fettsäuren im Plasma und der hohen Umsatzrate der freien Fettsäuren ist der Einbau der freien Fettsäuren in Plasmatriglyceride gegenüber Stoffwechselgesunden absolut erniedrigt; der prozentuelle Anteil der umgesetzten freien Fettsäuren, die in Triglyceride eingebaut werden, erreicht nur etwa ein Drittel der Norm.
4. Die Triglycerid-Clearance ist im Mittel erniedrigt, der Unterschied läßt sich jedoch statistisch nicht sichern. In der Triglycerid-Umsatzzeit besteht kein Unterschied.
5. Die niedrige Plasmakonzentration der Triglyceride bei Patienten mit Hyperthyreose ist trotz hoher Fettsäurekonzentration und erhöhtem Fettsäureumsatz eine Folge der verminderten Veresterung der freien Fettsäuren zu Triglyceriden in der Leber.
Summary
1. Turnover rates of plasma free fatty acids, triglycerides, triglyceride turnover time, and triglyceride clearance were determined in hyperthyroid patients and normals by means of constant infusion of palmitic-9,10-H3 acid.
2. In patients with hyperthyroidism the concentration of free fatty acids, the free fatty acid turnover rate, and the fractional turnover rate were elevated. A positive correlation was found between the concentration and turnover rate of free fatty acids in normals and patients with hyperthyroidism.
3. In spite of the high concentration and the high turnover rate of free fatty acids, the esterification of free fatty acids to plasma triglycerides (triglyceride influx) was reduced.
4. There was no difference in the clearance of triglycerides and the triglyceride turnover time.
5. The low concentration of plasma triglycerides in patients with hyperthyroidism is considered to be the result of reduced esterification of free fatty acids to plasma triglycerides in the liver.
