Zusammenfassung
1. Durch konstante Infusion von Palmitinsäure-9,10-H3 in Form eines Albuminkomplexes werden Umsatzraten der freien Fettsäuren und Triglyceride, Triglycerid-Umsatzzeit und Triglycerid-Clearance bei Patienten mit Insulin-bedürftigem Diabetes mellitus bestimmt und mit stoffwechselgesunden Personen verglichen. Außerdem werden diese Werte mit dem Blutzuckerspiegel in Beziehung gesetzt.
2. Die Umsatzrate der freien Fettsäuren ist bei Diabetikern erhöht. Die positive Korrelation zwischen Fettsäurespiegel im Plasma und Einbau der freien Fettsäuren in Plasmatriglyceride gilt für Normalpersonen, nicht aber für die untersuchten Diabetiker.
3. Wird bei diesen Patienten die Umsatzrate der freien Fettsäuren gegen den Blutzuckerspiegel aufgetragen, dann ergibt sich eine Verteilung der Werte in Form einer nach rechts offenen Parabel. Die Umsatzrate der freien Fettsäuren ist bei einem Blutzuckerspiegel um 200 mg-% am niedrigsten, bei höheren oder niedrigeren Blutzuckerwerten nimmt der Fettsäureumsatz zu.
4. Wird die Einbaurate der freien Fettsäuren in Plasmatriglyceride gegen den Blutzuckerspiegel aufgetragen, ergibt sich eine nach links offene Parabel: Der Einbau ist bei Blutzuckerwerten um 250 mg-% am höchsten und nimmt bei höheren oder niedrigeren Blutzuckerwerten ab.
5. Die Werte für die Triglycerid-Clearance und für die Plasmatriglycerid-Umsatzzeit unterscheiden sich nicht signifikant von der Kontrollgruppe.
6. Die Bedeutung dieser Befunde für die therapeutischen Maßnahmen beim Diabetes mellitus wird diskutiert.
Summary
1. The turnover rate of free fatty acids and plasma triglycerides, the clearance of plasma triglycerides and the plasma triglyceride turnover time are determined in normal persons and in insulin dependant diabetics.
2. In these patients the turnover rate of free fatty acids is increased. The positive correlation between concentration of free fatty acids and esterification of free fatty acids to plasma triglycerides applies to normals, but not to diabetics.
3. When the turnover rate of free fatty acids is plotted against blood sugar level, a parabolic distribution is noted such that at a glucose level of 200 mg-% mean free fatty acid turnover is lowest. At blood sugar levels both above and below this value free fatty acid turnover increases.
4. When the esterification rate of free fatty acids to plasma triglycerides is plotted against blood sugar level, again a parabolic distribution is noted such that the esterification rate is highest at a blood glucose level of 250 mg-% and decreases at higher or lower blood sugar levels.
5. The clearance and the turnover time of plasma triglycerides do not differ significantly in normals and diabetics.
