Zusammenfassung
Es wird über immunologische Untersuchungen bei einer Agranulocytose berichtet, welche durch eine Sensibilisierung gegen Pyramidon ausgelöst war. Der noch nach 15 Monaten nachweisbare Antikörper wurde charakterisiert. Er erwies sich chromatographisch und immunelektrophoretisch als ein IgG-Antikörper. Er reagiert nur mit menschlichen Leukocyten, bevorzugt Granulocyten, jedweder Blutgruppe. Zwei bei allergischen Arzneimittelcytopenien bisher unbekannte Phänomene konnten die täglich reproduzierbare Pyramidon-Leukoagglutination aufheben: die blutgruppenunabhängige Nichtagglutinabilität mancher Leukocytensuspensionen und die Inhibition von Pyramidon-Konzentrationen über 0,1 molar. Die Arzneimittelspezifität beschränkte sich im Gegensatz zu einem früher beschriebenen Antikörper auf zwei Phenazonstrukturen. Außer der Pyramidon-Leukoagglutination waren alle anderen serologischen Indicatorsysteme negativ.
Summary
Report of immunological studies on an agranulocytosis after sensitization by Pyramidon (aminopyrin). The antibody which was still found to be present after 15 months was shown, through chromatographic and immunoelectrophoretic methods, to be an IgG-antibody.
It reacts with human leucocytes, especially granulocytes regardless of bloodgroup. Two phenomena unknown in drug-induced hypersensitivity which could reverse the test in vitro were found:
1. The non-agglutinability of leucocyte suspensions taken from the same donor on different days.
2. The inhibition of the leuco-agglutination by Pyramidon of a concentration over 0,1 mol. The specificity of the antibody was limited to two phenazones, in contrast to that which was reported previously. Apart from the Pyramidon leuco-agglutination all other serological indicator systems were found to be negative.
