Zusammenfassung
Bei 11 Patienten mit arzneimittelallergischer Agranulocytose Schultz wurden die Serumkomplementspiegel (hämolytisches Komplement und β1A-Globulin) sowie die Konzentration der Immunglobuline im Serum bestimmt. Eine Hypokomplementämie ließ sich nur nachweisen, wenn die Komplementbestimmungen unmittelbar nach Einsetzen des Granulocytensturzes durchgeführt wurden. Nach Absetzen des Medikamentes wurde sehr rasch und bereits vor Normalisierung der Granulocytenwerte im Blut wieder Normokomplementämie erreicht. Eine Hyper-γ-Globulinämie des IgM-Typs war bei 4 Agranulocytose-Patienten auffällig. Ein Patient wies eine Paraproteinämie des Typs IgA auf. Es wird zur Diskussion gestellt, ob Veränderungen der Plasmaeiweißzusammensetzung im Sinne der Dys- bzw. Paraproteinämie den Ausbruch einer Agranulocytose Schultz begünstigen bzw. das Ausmaß der immunologischen Zellstörung beeinflussen können.
Summary
The sera of 11 patients suffering from drug allergic agranulocytosis were investigated for serum complement (hemolytic activity and β1A-globulin) and for serum concentrations of immunoglobulins. A marked but rapidly compensated decrease of serum complement was observed during the early stage of granulocytopenia when a test dose of aminopyrine was given to a patient sensitive to this drug. Hypocomplementemia is interpreted as a finding that supports indirectly the concept of immunopathogenesis of Schultz-disease. 4 patients with agranulocytosis were found to have hyper-gM-globulinemia, 1 had paraproteinemia. The question is discussed if certain disturbances of the plasma protein constellation may favour the outbreak of immunological cell destruction.
