Zusammenfassung
Biochemische und hämatologische Untersuchungen zur Pathogense der medikamenteninduzierten Anämie bei Glutathionreduktase-Mangelträgern erbrachten die folgenden Ergebnisse:

Unter Nitrofurantoinbehandlung fallen bei den Enzymdefektträgern die restliche Glutathionreduktaseaktivität und der GSH-Gehalt der Erythrocyten ab.

2.
Der zeitliche Ablauf der Anämisierung ist ähnlich wie bei Glucose-6-P-Dehydrogenase-Mangelträgern.

3.
Das Haptoglobin fällt nur mäßig im Serum ab, so daß die Ursache der Anämie weniger eine intravasale Hämolyse als eine vermehrte Sequestration der Erythrocyten im RES sein dürfte.

4.
Unter der Einwirkung von Nitrofurantoin, Chloramphenicol, Primaquin, Chloroquin und Phenacetin auf Enzyme der Glykolyse, des Hexosemonophosphatcyclus und der Glutathionreduktion nimmt nur die Aktivität der Glutathionreduktase ab.

Die stärkste Hemmung der Glutathionreduktase erfolgt durch Nitrofurantoin. Die Hemmung ist nicht kompetitiv. Die Hemmkonstanten für das gereinigte Enzym von Gesunden und Enzymmangelträgern sind gleich.

Die Hemmung der Glutathionreduktion durch Nitrofurantoin im Azoester-Test nachKosower wird dagegen nur in intakten Erythrocyten von Glutathionreduktase-Mangelträgern beobachtet. In normalen Erythrocyten tritt nur bei höherer Nitrofurantoinkonzentration eine geringe Hemmung ein.

Die Hemmung der Glutathionreduktion durch Nitrofurantoin bei Enzymdefektträgern ist bisher noch nicht sicher zu erklären. Als Ursache kommen eine verstärkte Permeation des Medikamentes durch die Erythrocytenmembran bei Defektträgern, eine vermehrte Bindung des Medikamentes an die Membran und lediglich bei den Enzymmangelträgern eine kritische Verminderung der Glutathionreduktion wegen der bereits verminderten Ausgangsaktivität der Glutathionreduktase infrage.
Summary
Biochemical and hematological tests in pathogenesis of druginduced anemia in glutathionereductase deficient patients produced the following results:

Nitrofurantoine treatment of enzyme-deficient patients leads to a decrease of the remaining glutathione-reductase activity as well as the GSH-content of erythrocytes.

Development of anemia resembles that of glucose-6-phosphate dehydrogenase deficient persons.

3.
Slight decrease of serum haptoglobine shows that increased sequestration of erythrocytes in the RES is more probably the cause of anemia than intravasal hemolysis.

4.
Influence of nitrofurantoine, chloramphenicol, primaquine, chloroquine and phenacetine on enzymes of glycolysis, hexosemonophosphate shunt and glutathione-reduction results exclusively in a reduction of glutathione-reductase activity.

5.
Nitrofurantoine is the most effective inhibitor of glutathione-reductase. Inhibition constants of the purified enzyme of normal and enzyme-deficient persons are identical.

Inhibition of glutathione-reduction by nitrofurantoine (azoester-test byKosower) is, in contrast, only observed in intact erythrocytes of glutathionereductase deficient persons. Normal erythrocytes require higher concentration of nitrofurantoine for slight inhibition.

No reliable explanation can yet be given for the inhibition of glutathione-reduction by nitrofurantoine. Possible causes may be: increased drug-permeation through the membrane of enzyme-deficient erythrocytes; increased binding of the drug on the membrane surface; critical decrease of glutathione-reduction due to already reduced enzyme-activity in enzyme-deficient persons.
