Zusammenfassung
Der Aldosteronstoffwechsel wurde bei Kranken mit kardialer Dekompensation verschiedener Ätiologie vor und zum Teil während Ödemelimination untersucht. Die Aldosteronanalysen wurden mit der Doppelisotopenmethode vonKliman undPeterson durchgeführt. Bestimmt wurden 1. Sekretionsrate (SR), 2. metabolische Clearancerate (=Plasmaclearancerate) (MCR) und 3. renale Exkretion (E). 4. Die mittlere Aldosteronplasmakonzentration ergab sich aus dem Quotienten SR/MCR. Gleichzeitige Analysen aller genannten Parameter konnten bei 15 Fällen von kardialer Dekompensation durchgeführt werden.
Eine erhöhte Aldosteronplasmakonzentration fand sich etwa in einem Drittel der Kranken; sie ließ sich entweder 1. auf eine Zunahme der Aldosteronsekretion oder 2. eine Abnahme der metabolischen Clearancerate oder 3. schließlich auf die Kombination beider Vorgänge zurückführen. Bei zwei Drittel der Fälle war kein Hyperaldosteronismus nachweisbar. Bei einigen dieser Fälle lag eine mäßige Verminderung der MCR vor. Eine normale Plasmakonzentration ergab sich durch eine relativ niedrige Aldosteronsekretionsrate.
Bei 9 von 11 untersuchten Kranken ließ sich im Ablauf der Rekompensation eine Zunahme der Aldosteronsekretionsrate nachweisen, während die metabolische Clearancerate keine gerichteten Änderungen erkennen ließ. Der gefundene induzierte Hyperaldosteronismus war bei den allein oder überwiegend saluretisch mit Spirolakton behandelten Kranken am stärksten ausgeprägt. Die durch Herzglykoside und/oder Thiazide erreichte Ödemelimination ging nur mit einem relativ geringen Anstieg der Aldosteronsekretion einher.
Als primäre Ursache des beobachteten induzierten Aldosteronismus wird die durch die saluretische Behandlung hervorgerufene Natriumexkretion angesehen. Die Aldosteronsekretion kann offensichtlich durch die unter Spirolakton entstehende Kaliumretention gesteigert und bei Behandlung mit kaliuretisch wirkenden Pharmaka gehemmt werden. Verschiedene Befunde lassen vermuten, daß der therapeutisch induzierte Aldosteronismus nicht über das Renin-Angiotensinsystem vermittelt wird.
Summary
Secretion rate (SR), metabolic clearance rate (MCR), mean plasma concentration (MPC=SR/MCR) and renal excretion (E) of aldosterone were determined by double isotope derivative assay (Kliman andPeterson) in patients with congestive heart failure of various etiology.
Mean plasma concentration of aldosterone (MPC) was found to be increased in 6 out of 15 cases studied. This increase was due to (1) either an increase of aldosterone secretion (SR), (2) a decrease of the metabolic clearance rate (MCR) or (3) both (1) and (2). No hyperaldosteronism was discovered in approximately 2/3 of our patients. Some of those cases revealed reduction of MCR; normal plasma concentration (MPC) resulted from relatively low aldosterone secretion (SR).
From these results it can be derived that hyperaldosteronism is apparently not essential in sustaining cardiac edema. It cannot be excluded however, that in an earlier phase of congestive heart failure aldosterone is a significant factor in the pathogenesis of sodium retention.
Hyperaldosteronism is typically shown to be a consequence of saluretic therapy: In 9 out of 11 cases studied an increase of SR could be observed in the course of edema excretion, whereas MCR proved to be inconsistent. This induced hyperaldosteronism seemed to be pronounced in patients treated with spironolactone. The increase of SR in patients treated with thiacides was only slight.
While increased sodium excretion is considered to be the cause of the stimulation of aldosterone secretion, further increase of SR is likely to occur as a consequence of potassium retention as produced by the action of spironolactone. The stimulatory action of negative sodium balance on aldosterone secretion can possibly be suppressed by increased potassium excretion. Various findings suggest that induced hyperaldosteronism in congestive heart failure is not mediated via the renin-angiotensin system.
