Zusammenfassung
25 sonst stoffwechselgesunden Magenpatienten in einheitlichem Ernährungszustand wurden nach mindestens 7tägiger Einstellung auf eine orale Standard-Diät, deren Stickstoff- und Caloriengehalt denen der nachfolgenden parenteralen Ernährungsperiode angenähert war, mehrtägig durch i.v. Infusionen einer synthetisch zusammengesetzten Aminosäurelösung unter Standardbedingungen ernährt, wobei zwölf dieser Patienten zusätzlich in einer zweiten Hälfte der entsprechend verlängerten Vorperiode und in der Infusionsperiode täglich 3 × 5 mg Dianabol® erhielten.
Die N-Bilanzen sind unter der Standard-Diät und der parenteralen Ernährung auf dem gleichen Niveau negativ und werden durch die zusätzliche Dianabol-Medikation nach einer Latenzzeit von ca. 24 Std signifikant um durchschnittlich je +2 g N/Tag verbessert.
Der Anstieg der N-Retention unter der Anabolicum-Medikation in unserer Infusionsperiode beruht ausschließlich auf einer absolut und relativ verminderten Ausscheidung von Harnstoff-N. Die daraus zuerrechnende täglich weniger katabol metabolisierte Aminosäuremenge (ca. 17 g) entspricht energetisch etwa der parallel gemessenen Verminderung des täglichen Energieumsatzes (ca. 80 kcal), ein unmittelbarer Zusammenhang zwischen den beiden Meßwerten erscheint denkbar.
Die α-Amino-N-Ausscheidung in der Vorperiode und die Ausscheidung freier Aminosäuren in der Infusionsperiode wird durch die Dianabol-Medikation kaum beeinflußt. Änderungen der Metabolisierungsrate von Glycin und oder D-Aminosäuren können somit als Ursache der vermehrten Proteinsynthese weitgehend ausgeschlossen werden.
Obgleich bis heute die genaue Wirkungsweise anaboler Hormone und die Lokalisation dieser Wirkung beim Menschen unbekannt sind, scheint die Anwendung anaboler Hormone unter qualitativ und quantitativ ausreichender Eiweißzufuhr bei geeigneten Fällen auch im Rahmen der parenteralen Ernährung sinnvoll.
Summary
25 patients suffering from peptic ulcers or gastritis but without showing any metabolic disturbances were fed for at least 7 days on a preliminary oral standard diet whose N and caloric content resembled the subsequent parenteral nutrition. They were then fed under standard conditions for several days by intravenous infusions of a synthetic mixture of pure crystalline amino acids. During the second half of the preliminary oral diet and during the i.v. infusion period, 12 of the patients were given, in addition, 3 × 5 mg Dianabol® (17 α-Methyl-17 β-hydroxy-androsta-1,4-dien-3-on) daily.
The N-balances are equally negative under both standard diet and parenteral nutrition, and are, after a lag period of ca. 24 hrs., significantly improved (on average + 2 g N/day) by the administration of Dianabol.
The improvement of N retention under the influence of the anabolic steroid during the i.v. infusion period is due exclusively to an absolute and relative lowering of urea N excretion. From this value can be calculated the decreased amount of amino acids catabolised daily (ca. 17 g), which corresponds in energy content to the decrease in daily energy conversion (ca. 80 kcal) which was measured in parallel. A direct relationship between the two sets of results therefore seems possible.
The α-amino-N excretion during the preliminary oral diet period, and the excretion of free amino acids during the infusion period are hardly influenced by the administration of Dianabol. Changes in the rate of metabolism of glycine and/or D-amino acids can, to a large extent, be excluded as a cause of increased protein synthesis.
Although the exact mode and sites of action of anabolic steroids in man are unknown, it seems that their administration in suitable cases, under qualitatively and quantitatively sufficient protein supply, can be useful in the field of parenteral nutrition.
