Zusammenfassung
Es wird der Einfluß einer ungleichmäßigen Verteilung des Ventilations-Perfusionsverhältnisses in der funktionell inhomogenen Lunge auf die Arterialisierung des Blutes unter Berücksichtigung von theoretisch ermittelten und experimentell bestimmten Verteilungskurven untersucht. Die theoretische Untersuchung liefert folgende Ergebnisse:

Eine ungleichmäßige Verteilung des Ventilations-Perfusionverhältnisses führt zu einer gleichartigen regionalen Variation des Respiratorischen Quotienten.

2.
Als Folge des Bohr-und Haldaneeffektes ergibt sich die Existenz eines kleinstmöglichen RQ-Wertes von 0,3 und eines größtmöglichen RQ von 6–10.

3.
Bei einem Mittelwert von RQ=0,8 ist in der funktionell inhomogenen Lunge mit dem Vorliegen einer linksschiefen Verteilung zu rechnen.

4.
Unter Berücksichtigung verschiedener, in pathologischen Fällen möglicher linksschiefer RQ-Verteilungen errechnen sich die verteilungsbedingten alveolar-arteriellen O2-Druck-differenzen (AaD) zu 15–30 Torr.
Die experimentelle Untersuchung der Verteilung des Ventilations-Perfusionsverhältnisses mit Hilfe des „inspiratorischen Sauerstoffsprunges“ nachThews undSchmidt an 8 Emphysematikern führt zu folgenden Ergebnissen:

Die experimentell gewonnenen Verteilungskurven entsprechen nach Form und Verteilungsdichte den theoretisch begründeten Kurven.

2.
Die hieraus ermittelten verteilungsbedingten alveolar-arteriellen O2-Druckdifferenzen (AaD) liegen im Bereich von 18–32 Torr.

3.
Die Durchblutungsverteilung zeigt eine der Ventilationsverteilung angepaßte Tendenz, so daß bei Vernachlässigung der Inhomogenität der Durchblutung eine um etwa 30% zu große AaD berechnet wird.

4.
Die alveolare Totraumventilation kann beim ausgeprägten Lungenemphysem 20–70% der Gesamtventilation betragen.
Summary
Unequal distribution of ventilation/perfusion ratio in the functionally inhomogeneous lung was studied as to its effect on the arterialization of blood; this was done in consideration of theoretically found and experimentally determined distribution curves.
The following results were obtained by the theoretical study:

1.
Unequal distribution of ventilation/perfusion ratio yields regional variation of the respiratory quotient.

2.
The existence of a smallest possible RQ value of 0,3 and of a highest possible RQ of 6–10 is the consequence of the Bohr and Haldane effect.

3.
Leftwardly inclined RQ distribution may be assumed in the functionally inhomogeneous lung at a mean value of 0.8.
-
4.
In consideration of different leftwardly inclined theoretical RQ distributions which might be possible in pathological cases, the alveolar-arterial O2-pressure differences (AaD) are calculated to be 15–30 mmHg.
In eight emphysematous patients the distribution of ventilation/perfusion ratio was studied experimentally by continuous recording of alveolar and arterial oxygen pressures during sudden change of inspiratory oxygen concentration in hyperoxia. These experiments had the following results:

Shape and density of distribution curves obtained experimentally are similar to those obtained from theoretical studies.

2.
The alveolar-arterial O2-pressure differences (AaD) resulting from unequal distribution range from 18–32 mmHg.
-
3.
The distribution of perfusion tends to adapt to that of ventilation, so that when ignoring the inhomogeneity of perfusion AaD will be calculated too high by about 30 per cent.

4.
Alveolar dead space ventilation may amount to 20–70 per cent of total ventilation in pronounced pulmonary emphysema.
