Zusammenfassung
Mit der radialen Immunodiffusionsmethode nachMancini wurden die IgG-, IgA-und IgM-Konzentrationen bei 55 Patienten mit verschiedenen Lebererkrankungen bestimmt. Die dabei erhaltenen Ergebnisse sind in Tabelle 4 zusammengestellt.
Das unterschiedliche Verhalten der Immunglobulinkonstellationen zwischen der akuten Hepatitis und der Lebercirrhose, vor allem der IgG-, IgA- und IgM-Konzentrationen ist signifikant.
Bei der akuten Hepatitis nehmen die anfänglich stets erhöhten IgM innerhalb von 2–4 Wochen deutlich ab. Es besteht eine positive Korrelation zwischen der Stärke der Mesenchymproliferation und der IgA-und IgG-Konzentration sowie der Höhe der IgG/IgM-und IgA/IgM-Quotienten. Zwischen dem Ausmaß der Leberzellnekrosen und den Immunglobulinen sowie zwischen IgM-Konzentration und Thymolwert läßt sich keine Beziehung herstellen, ebensowenig zwischen dem Ig-Spiegel und der SGOT- und SGPT-Erhöhung. Die Veränderungen der Ig sind nicht leberoder krankheitsspezifisch. Die Untersuchung der Ig gibt jedoch im Rahmen des klinischen Gesamtbildes wertvolle Hinweise bei differentialdiagnostischen Überlegungen und für die Verlaufskontrolle zahlreicher akuter und chronischer Lebererkrankungen, besonders der Hepatitis und ihrer Folgezustände.
Summary
IgG-, IgA- and IgM-concentrations were determined in 55 patients with different diseases of the liver by means of the radial immunodiffusion test ofMancini. The findings observed are summarized in Table 4.
Differences of Ig-constellations are most relevant between acute hepatitis and cirrhosis of the liver. The different behaviour of IgG-, IgA- and IgM-concentrations in these groups proves to be significant. In acute, active hepatitis the initially high IgM-values tend to decrease within 2–4 weeks. A positive correlation is found between the degree of mesenchymal proliferation and IgA- and IgG-concentration on one hand and the height of IgG/IgM- and IgA/IgM-ratio on the other. No relationship is found between the extent of liver cell necrosis and the Ig; there is also no correlation between IgM-concentration and the thymol turbidity test, nor between immunglobulin level and increase of SGOT and SGPT. The changes in Ig-concentrations in human blood must not be thought to be specific or diagnostic for any form of liver disease, but of high value, if beeing used together with the whole clinical and other biochemical diagnostic arsenal. Knowing their behaviour is most valuable for differential diagnosis and for observing the course and the tendency of the diseases, especially of hepatitis and its sequels.
